About this file
This document is a Proprietary Information, Qualification Questionnaire, and Mandatory Requirements form from the City of Fort Worth Purchasing Division for a Request for Quotation (RFQt) for Pipe Plugs, with the reference number 25-0205. The document is designed to gather detailed information from potential bidders and outline the requirements for a responsive proposal. The RFQt seeks to qualify vendors who can provide pipe plugs and requires bidders to complete a comprehensive questionnaire covering topics such as cooperative agreement availability, payment terms, company history, contact information, and operational capabilities.
The document emphasizes key submission requirements, including the need to complete specific forms such as RFQt-02 Form CIQ, RFQt-03 Authorized Signed and Requirements, and the RFQt-07 Questionnaire. Bidders must also be prepared to address proprietary information protocols, with clear guidelines that all submitted materials become public property subject to the Texas Public Information Act. The City requires vendors to specify their ability to meet delivery timelines (5 business days), expedite parts, provide business hours, and confirm they can fulfill all specified vendor service requirements. Pricing details and additional service discounts are to be outlined, with a specific note that pricing information cannot be marked as proprietary and will be publicly available.
