About this file
This is an MBE Utilization Form submitted by the City of New Haven in Connecticut for documentation of subcontractor participation in a project related to direct commodity purchases for the New Haven Public Schools Child Nutrition Program. The form requires contractors to submit a complete listing of all subcontractors to be used on the project, including company names, addresses, types of work to be performed, dollar amounts allocated to each subcontractor, outreach methods used to secure the subcontractors, and appropriate company classifications. The City of New Haven has established a goal of 25% participation from minority-owned businesses (MBE) and women-owned businesses (WBE). Contractors must certify that they have reviewed the city's MBE and WBE goals and have achieved the required 25% participation threshold. Submission of the completed form is mandatory, and failure to complete and submit the form will result in the bid being deemed non-responsive and rejected.
The form serves as a compliance and documentation mechanism for the procurement of commercial and commodity food items for the New Haven Public Schools Child Nutrition Program, which serves approximately 17,500 to 19,000 meals per day across approximately 224 service days during the 2026 summer year and 2026-2027 school year. Contractors must provide detailed information about each subcontractor's involvement, including the outreach methods employed to identify and engage minority and women-owned businesses, ensuring transparency and accountability in meeting the established diversity participation goals for this procurement opportunity.
