The form must be completed and returned by bidders as part of their proposal submission for the Mattress Cores procurement (Solicitation Number DQ 2027000046). Contractors are required to answer whether any services or subcontracted services will be performed outside the United States or Colorado, and if affirmative, must specify the type of services, geographic location(s), and justification for performing work outside these boundaries.

The form serves as notice to the Department of Personnel of any contract awarded to a vendor performing services outside U.S. or Colorado borders, in compliance with Colorado Revised Statutes 24-102-206 and 24-102-205. Contractors must provide written notification within 20 days of contracting or deciding to contract outside the specified geographic area.
