About this file

This is a Vendor Disclosure and Notification form (Exhibit E) issued by the Colorado Department of Corrections (CDOC) for the State of Colorado, required in compliance with Colorado Revised Statutes 24-102-206. The form must be completed and returned with bids for any contract or subcontract that involves services performed outside the United States or the State of Colorado. Vendors are required to disclose whether services will be performed outside these jurisdictions and, if so, must specify the types of services, locations where services will be performed, and the business justification for performing work outside the United States or Colorado. The form designates that completion should be performed by the business's President, Board Chairperson, or authorized Contract Signatory, with submission directed to the Sole Contact listed in the solicitation.

The form includes a procurement use section requiring documentation of the solicitation number, contract type, and verification of whether the vendor provided notice within 20 days of deciding to contract services outside the United States or Colorado, as mandated by statute. This disclosure requirement applies to non-construction contracts and serves as official notice to the Department of Personnel of any out-of-jurisdiction service performance arrangements. Vendors must answer a yes or no question regarding anticipated out-of-jurisdiction services and complete supplementary questions accordingly.
