About this file

This document is a Texas Family Code form related to state contract bidding requirements, specifically focusing on child support payment compliance. The document outlines Section 231.006, which prohibits sole proprietorships, partnerships, corporations, or other business entities from bidding on or receiving state contracts if any owner (sole proprietor, partner, majority shareholder, or 25% or more business owner) is 30 days or more delinquent in child support payments under a court order or written repayment agreement.

The form requires bidders to complete an affidavit certifying compliance with this requirement, including providing the name and social security number of the individual or business entity and all partners, shareholders, or owners with at least 25% ownership interest. The document emphasizes that inaccurate certification may result in contract termination and withholding of payments. The form is part of RFP #26-002 and was revised in February 2014, serving as a mandatory compliance document for entities seeking state contracts in Texas.
