About this file
This is Addendum No. 2 to a Request for Proposals issued by the City of Chandler, Arizona for Occupational Health Care Provider services under solicitation number HR5-948-4846. The addendum, released on September 27, 2024, addresses frequently asked questions and clarifies service requirements for the City's approximately 2,200 full-time and part-time employees. The City anticipates approximately 150 claims requiring occupational health services annually, based on current year projections. The addendum confirms that Physician Assistants and Nurse Practitioners are acceptable to fulfill service requirements under physician supervision, and that physical therapy authorization is managed collaboratively between the medical director and claims adjuster rather than requiring pre-authorization. A section numbering correction is noted, moving Additional Services Upon Request from Section 5 to Section 6 of the original RFP documentation.
The addendum provides detailed specifications for drug testing panels offered under the contract. The 9-Panel (DOT) test and 12-Panel tests are outlined with specific compounds tested, including controlled substances such as amphetamines, benzodiazepines, cocaine metabolites, marijuana, opiates, and phencyclidine. The 12-Panel includes additional compounds such as hydromorphone, oxycodone, oxymorphone, and fentanyl. The addendum clarifies the distinction between SAMHSA-regulated panels and non-SAMHSA panels, noting that SAMHSA panels comply with federal workplace drug testing regulations while non-DOT testing allows for customizable testing parameters. Prospective vendors must acknowledge receipt of this addendum in their proposals.

OCCUPATIONAL HEALTH CARE PROVIDERS
Solicitation No. HR5-948-4846
Addendum No. 2
This Addendum No.2 is released to clarify frequently asked questions. If any provision in this addendum conflicts with any existing provisions in the RFP, the provisions of this addendum control.
All other provisions in this RFP remain effective as previously published.
Question 1: What is the projected volume of services for the contents of this RFP?
Answer 1: We have approximately 2200 full time and part time employees at the City of Chandler.
We had 133 claims that required some level of care in 2023. We have had 150 claims so far this year.
Question 2: Can you please tell me if Physician Assistants and Nurse Practitioners can fulfill the intent of the RFP. We have a Doctor that is our Medical Director and will see all Az Post patients, but we also employ NP's and PA's that would be seeing the injuries and the NFPA patients.
Answer 2: Yes, this would be acceptable.
Question 3: Is authorization required prior to starting physical therapy?
Answer 3: No, the doctor works with our claims adjuster to approve therapy. We don't determine treatment.
Question 4: Can you provide the drugs included in each of the panels 9-13? (Pg. 17 Pricing Sheet)
Answer 4: Please see attached Drug Testing Compound List
Question 5: What is the difference between a Non-SAMHSA and a SAMHSA panel? (Pg. 17 Pricing
Sheet)
Answer 5: SAMHSA is the federal agency that regulates workplace drug testing. We utilize these regulations for our testing. The difference is in the DOT (Department of Transportation) has different requirements than the Non-DOT test which we can determine what we test for.
