Item 1 (Contract ID Code). Insert the contract type identification code that appears in the title block of the contract being modified.

For a solicitation amendment, change order, or administrative change, the effective date shall be the issue date of the amendment, change order, or administrative change.
For a supplemental agreement, the effective date shall be the date agreed to by the contracting parties.
For a modification issued as an initial or confirming notice of termination for the convenience of the Government, the effective date and the modification number of the confirming notice shall be the same as the effective date and modification number of the initial notice.

Q: Thermostat and U.S. District Court Seal: Are these items confirmed for removal and reinstallation after the reupholstery work?
A: Confirmed.
Q: Additional Appurtenances: Besides the wall panels, thermostat, and court seal, are there any other items on or near the wall that require removal and reinstallation?
A: There is one thermostat, one fire alarm strobe, and one receptacle cover to be removed and reinstalled.
Q: How many total electrical fixtures (i.e. thermostat, outlets, etc.) will need to be removed to perform the work?
A: Same as the previous question.
Q: Are the panels above & below the white board 2 separate panels or one continuous panel?
A: Two separate panels.
Q: Where can we park to perform the work?
A: One parking space will be provided at the building parking lot/loading dock.
Q: What is the distance, including # of floors, between the court room and the parking area?
A: 5 Floors via elevator
Q: Are there any specified fabrics?
A: No, contractor is requested to match existing fabric color and texture as best as possible.
Q: If the structure of a panel requires repair, can any additional labor/materials charges be added after entering contract?
A: If there is any unforeseen work that is necessary outside of the already identified requirements of the Scope of Work (SOW), then you would need to notify the Contracting Officer’s Representative (COR) and Contracting Officer prior to completing the additional work so that any necessary contract modification could be discussed.
