EQUIPMENT SURVEY & SAFETY STUDY - FINAL REPORT

December 14, 2018

Revision 1, December 20, 2018

1.0 OVERVIEW OF EXISTING BUILDING

The U.S. Courthouse is located at 131 East Fourth Street, at the southwest corner of Perry and East Fourth streets in downtown Davenport, Iowa. The U.S. Courthouse is a 79,853 GSF, 3-story federal building with a structural steel frame and concrete floor slabs. The federal building has a nearly rectangular footprint, which results in a box-like form with a flat roof. The exterior walls are clad in a warm beige Minnesota Kasota Travertine limestone ashlar with a base of dark gray polished granite. The building is detailed with stripped-down Classical elements such as fluted carvings and low relief spandrels.

The U.S. Courthouse was originally constructed from 1932-1933 as a post office and courthouse. After the first renovation in the 1960s, which converted the former postal facilities into offices, there were additional renovations during the 1970s and 1990s. Between 2003 and 2005, the building was renovated under GSA's Design Excellence program. The project, which converted the building to a dedicated courthouse facility, included the construction of two new courtrooms, restoration of the historic courtroom, and renovation of the first-floor lobby

The principal facade faces north onto East Fourth Street and is the most elaborate of the four elevations.

This elevation is nine bays wide on the first story, with two entrances. The south elevation, facing a public alley, is utilitarian in character. The east elevation, facing Perry Street, and west elevation, facing a private alley, are nearly identical. Lacking entrances, these elevations are less elaborate than the principal elevation. A recessed loading platform occupies a seven-bay wide area of the elevation, resulting in a U-shaped footprint with two wings. The building contains courtrooms, judge’s chambers, 4 elevators, (1 judge’s,1 prisoner, 1 public, and one 2-stop freight [i.e., basement to 1st floor]) with surface parking.

Building Code Information

Use and Occupancy Classification:

Mixed use occupancy consisting predominantly of business, assembly and ordinary contents storage uses. A non-separated mixed-use occupancy approach is permitted for this building.

Construction Classification:

NFPA Type II (222); IBC Type IB 3 stories plus basement, two mezzanine levels and the roof.

Fully sprinklered. Rated corridors are not required per NFPA 101, 38.3.6.1(3) for business and 12.3.6(3) for assembly.

The Davenport U.S. Courthouse is arranged with three stairwells.

2.0 PROJECT SUMMARY

In January 2018, the GSA field office notified the Regional Safety Specialist that multiple pieces of equipment do not have safe access for regular maintenance. The Regional Office issued a work order to perform a field survey to document safety deficiencies throughout the building and develop corrective actions.

In June 2018, Etegra, Inc. performed a site survey along with Haines Janson & Associates safety consultants. Several deficiencies were documented and discussed with GSA. Additional field work was

December 20, 2018 Page 3 performed in October 2018 to identify several additional items and to gather information to further develop the corrective actions.

3.0 CORRECTIVE ACTIONS SUMMARY

Sixteen individual items have been identified for corrective action. These are classified as walking/working surface items. Corrective actions will provide appropriate safe access to the equipment.

A spreadsheet of deficiencies can be found in Appendix A. Each item is identified with a unique finding number. The abbreviations used identify the type and general location of the deficiency.

Example: xx-OSH-##

First letter/number group: Floor Level. R indicated roof level. 1M and 3M are mezzanine maintenance levels located above the first and third floors, respectively.

Second letter group: Deficiency type. OSH = Occupational Safety & Health Deficiency Third group: Sequential numbering of deficiencies found. Numbers may be skipped, as some items have been removed from the final scope of work.

Appendix B contains engineering judgment pages for each deficiency, including a photograph of the location and a recommended corrective action.

4.0 SUMMARY SCOPE OF WORK

Appendix C contains drawings identifying the location of each deficiency using the finding number.

Additional drawings are included for some items to describe the suggested corrections. Below is a narrative description of the work required for each corrective action.
