About this file

This document is an attachment to a state contract for Group 40061 Protective Outerwear, specifically a certification form related to Executive Order No. 16 regarding business operations in Russia. The certification is required for vendors seeking to enter or renew state contracts, focusing on prohibiting state agencies from contracting with businesses conducting business in Russia.

The contract appears to be a statewide vehicle for firefighting, emergency/rescue, and safety protective outerwear. The attachment requires vendors to self-certify their business operations in Russia by selecting from three possible categories: not conducting business in Russia, winding down Russian operations, or conducting limited essential services. Vendors must complete the certification under penalty of perjury, providing details about their business operations and confirming their compliance with the executive order's restrictions on doing business with Russian entities.
