About this file

This document is a Questionnaire and Required Documentation form from the Parks and Recreation Department for a bid submission. The questionnaire requires potential bidders to provide comprehensive information about their company, including authorized contacts, company history, key personnel, available equipment, workforce capacity, and past contract experience. Bidders must detail their quality assurance processes, anticipated daily service schedule, and confirm their ability to provide the required insurance coverage.

The questionnaire is designed to thoroughly evaluate bidding firms' qualifications and capabilities for the contract, requesting detailed information across ten specific areas. Bidders are expected to provide extensive documentation including biographies of principal and key personnel, a list of owned equipment, comparable contract history from the past three years, quality control methodologies, and a tentative operational schedule. A critical requirement is the ability to obtain a certificate of insurance with specified coverages and limits, which is considered a major contractual prerequisite.
