About this file

This is a Past Performance Questionnaire template for solicitation N00244-26-Q-0056 related to flight suit manufacturing services. The questionnaire is designed to be completed by contractors and their previous clients to evaluate the contractor's past performance during source selection. Contractors must submit one questionnaire per relevant contract performed within the five years prior to solicitation issuance, with relevance determined by similarity in product complexity, manufacturing scope, and contract magnitude. The contractor completes Section I with contract identification details including contractor name, CAGE code, contract number, work description (types of flight suits manufactured, materials used, quantities delivered, measurement and fitting requirements), period of performance, total contract value, and client point of contact information.

The client/respondent completes Sections II and III, rating the contractor's performance across Quality of Product (conformance to technical specifications, accuracy of tailoring, workmanship quality, and material authenticity), Schedule & Delivery (timeliness of deliveries and adherence to turnaround times), Contract & Business Management (communication, problem resolution, flexibility, and invoicing accuracy), and Overall Performance. Respondents must also provide narrative assessments of contractor strengths, weaknesses, corrective actions taken, and whether they would award another contract. Completed questionnaires must be submitted directly to Government Points of Contact Queen Idele (Contract Specialist, queen.e.idele.civ@us.navy.mil) and Kyle Westover (Contracting Officer, kyle.d.westover.civ@us.navy.mil) no later than 4:00 PM Pacific Daylight Time on April 30, 2026. Contractors are strictly prohibited from receiving completed questionnaires from their clients, and violation of this requirement may result in quotation ineligibility.

Past Performance Data Questionnaire

PURPOSE:

This questionnaire is a key part of the Government's evaluation process for the solicitation referenced above. The information you provide will be used to evaluate the past performance of the contractor listed in Section I below. Your candid and timely feedback is greatly appreciated.

Offerors shall complete one (1) Past Performance Questionnaire per contract/order. The Government reserves the right to verify information provided and to contact references.

INSTRUCTIONS

To the Contractor (i.e., the company quoting on the new contract):

1. Completely fill out Section I of this form for a recent and relevant contract you have performed. A separate form is required for each past performance reference submitted.

a. Recent means the contract is currently active or was completed within five (5) years prior to the issuance date of this solicitation.

b. Relevant means the contract demonstrates similarity in the areas of product complexity, manufacturing scope, and contract magnitude.

2. Send this entire form (with Section I completed) to your point of contact (POC) at the client organization for that contract.

3. Instruct your client POC to complete Section II and Section III and submit the form directly to the U.S. Government Contracting Team listed below. To ensure the integrity and confidentiality of the past performance information, the contractor is strictly prohibited from receiving the completed PPQ from their client. Under no circumstances shall the completed questionnaire be returned to the contractor. A violation of this direct submission requirement may compromise the integrity of the evaluation process and may result in the contractor’s quotation being deemed ineligible for award.

To the Respondent (i.e., the Contractor’s previous client):

1. Please complete Section II and Section III of this questionnaire.

2. Your assessment should reflect the performance of the contractor identified in Section I on the referenced contract.

3. Completed questionnaires shall be submitted via email directly to both Government Points of Contact (POCs) identified below. Receipt of the completed questionnaire by the Government POCs is required no later than 4:00 PM Pacific Daylight Time (PDT) on April 30, 2026. Questionnaires received after this date and time are considered late and may not be evaluated. The information you provide will be treated as Source Selection Sensitive information and will not be disclosed outside of the evaluation team.
