About this file
This is a Foreign Country of Concern Attestation form (PUR 1355) required by the State of Florida for entities bidding on or entering into contracts with governmental entities that would grant access to personal identifying information. The attestation must be completed and signed by an officer or representative of the bidding entity. The form requires the entity to declare under penalties of perjury that it is not owned by the government of a Foreign Country of Concern, is not organized under the laws of a Foreign Country of Concern, does not have its Principal Place of Business in a Foreign Country of Concern, and that the government of a Foreign Country of Concern does not hold a Controlling Interest in the entity.
The attestation applies to Halifax Health's procurement for NetApp Storage Hardware, which involves high-performance, scalable storage solutions with integrated data protection, hybrid cloud connectivity, and comprehensive support services based on NetApp ONTAP architecture. Bidders must complete the form with printed name, title, signature, and date, adhering to the definitions provided in Rule 60A-1.020, Florida Administrative Code. This compliance requirement ensures that vendors meeting foreign ownership restrictions are eligible to handle sensitive personal information in connection with the contract.
