This amendment is issued to update the last paragraph on the SF1449. Each capsule shall contain a minimum of 500mg of EPA/DHA. Any combination of EPA/DHA is acceptable provided both EPA and DHA are present in each capsule and the combined total of EPA/DHA is equal to or greater than 500mg/capsule.
Offers are still due December 10, 2021 at 2:30PM Central Time

To be considered for award, offerors must submit a price for line item: 1 for the base year and all four option years. One award will be made for line item: 1 for the base year and all four option years. Offered prices shall not exceed two decimal places.
Offeror must provide documentation that the product is certified by a company such as United States Pharmacopeia (USP) Verified, ConsumerLab (CL) Approved, NSF International NSF Mark, National Products Association (NPA), TruLabel or other which (1) provides a physical inspection of the manufacturing site and assurance that the product was properly manufactured, (2) that it contains the ingredients listed on the label in the stated amounts, and (3) that does not contain harmful contaminants. Certification must be maintained throughout the contract period. The certifications must be submitted by the due date for offers. In order to be eligible for award, offerors must submit an acceptable certification as described above.
All unit of use packaging identified in the Schedule of Supplies, shall contain a safety-cap capable of having the outer part of the cap deshelled and / or a convertible cap to produce a non-safety cap and a bottle with a minimum required size of 120cc volume. The safety cap must not exceed the diameter of the bottle. The bottle must have a cylindrical body and minimum of 5.75” circumference. The bottle must fit a label with a dimension of 5.25” x 2”. The 0.5” additional space is required to keep the bar code exposed and/or allow for the display of the expiration date and lot number.
All bottles must contain a grade A safety seal under the cap. The label must be absent of any indication for use language.
Product Samples. Offerors shall provide one sample (one bottle) for line item 1. Samples will not be returned. (See FAR Clause 52.212-1(d)). The samples shall be delivered to:

Offerors shall provide a tracking number by email to Youlanda.Duggar@va.gov once they have it. The samples must be received by the due date for offers. Samples that are received after the due date/time will result in that offer being rejected and shall receive no further consideration. Any offer whose proposed samples fail to meet the minimum requirements shall be rejected and shall receive no further consideration.
