SOLICITATION # 36E77619B0015 PROJECT NAME: Salisbury 5th floor Medical Surgical Floor

TECHNICAL QUESTIONS AND VA RESPONSE TRACKING SHEET

ITEM

NO. QUESTION GOVERNMENT RESPONSE

1. What is the approved path for removal of both abated materials and demolition as well as bringing new materials into the building for phase 1 work? There does not appear to eb access to the elevators.

There is access to a service elevator. However, all demoed material should be trash shoot to a dumpster. And material is usually boomed through a window.

The elevator is not a main path.

2. Will specific elevator be designated for demolition and materials being brought into the building?

NO

3. Will stocking of the job and removal of materials be designated as “OFF HOURS” work? If this is correct, provide hours for this to occur

Not necessarily, however if ICRA becomes an issue, contractor can be banned from using the elevator.

4. Can pre-bid meeting notes be issued? All questions were to be submitted in writing so there are no “notes”.

5. Can exterior window(s) be removed for trash removal? Provide location of this? If window can be removed, are the windows interior or exterior glazed?

Yes, interior glazed. Location will be provided after award during mobilization coordination.

6. Provide distance from 5th floor to grade

46’ estimated, contractor to confirm

7. Provide location of trailer / dumpster area for GC use.

TBD

8. Define ICRA level to be adhered to during the construction period. See Project Manual.

9. What is the existing deck height above? 12’ estimated, contractor to confirm

10. Documents reference “Appendices”? Please review and provide these for review and distribution In the Spec Book the word “Appendix” appears twice; once in section 028211- 6 which is a reference to code book, the second in section 280500-21 which is in reference to the TAB report that require and its “appendices”. Both references are of the contractor’s responsibility.

11. Interior signage specification is not found. Please advise and issue as required

N/A

12. Mechanical drawing M104, note 6 calls for concrete mechanical pad to reduce unit vibration transfer to building. What is specification for concrete. Note that this will be extremely expensive to get onto the roof as it probably will need to be pumped. Can mechanical engineer provide unit with additional vibration isolation and curbing to eliminate and thereby remove concrete from specification? This unit also was one of the units which I addressed for additional structural supports per the earlier email.

See Note 6 on page M104. Concrete Pad or Manufacturers’ requirements.

Pumping 2.6 cu ft of concrete.

13. Sheet G002:

o Note 21 refers to locker room bench. Provide both locations and specification o Note 25 calls for 24 x 36 white board. Provide locations and basis of design specification.

TBD by Interior Design and usually provided

14. Headwall / millwork questions:

o Is edge-banding required at cabinetry?

o Drywall soffit runs into cabinetry – assuming we run our cabinetry to the higher height and ceiling contractor interfaces with our headwall panel? Or can the soffit back up and start at end of cabinet? (NOTE: see picture above moving face of soffit back.)

Headwall provided by the VA and installed by Contractor. Contractor is to coordinate, supply and install all med gas, valves, electrical connections and devices and work boxes associated with and part of the headwall.

Millwork to have edge banding.

15. Assuming laminate for headwall material? Is this correct? If correct specific laminate selection required

Headwall provided by the VA and installed by Contractor. Contractor is to coordinate, supply and install all med gas, valves, electrical connections and devices and work boxes associated with and part of the headwall.

Millwork to have edge banding.

16. • Roofing (Approx. 14-16 months ago there was a building reroof project bid). Note the following questions:

o Did the re-roof of the building occur ? If so, do you know who the contractor for the roof was that did the work and who is manufacturer of the materials?

o If known, does the owner have a warranty? By NC law the Roofing Contractor has 2 years of liability for the roof, it would be improper for any other roofer to do work on the building of it is within the 2-year period, we also would need to know the holder of the Roofing warranty to make sure we could do the work under the terms of the coverage.

• Roofing request for alternate roofing: Roofing per Division 7, Section 07 54 16 as currently specified is Fibertite unit and is proprietary per ASTM 6754. Can alternate roof similar to Soprema Sentinel P150 be utilized per ASTM 4434 and attached information? Note the following:

o Proposed substitution has been fully investigated and determined to be equal or superior in all respects to specified product.

o Same warranty will be furnished for proposed substitution as for specified product.

o Same maintenance service and source of replacement parts, as applicable, is available.

New roof has been placed. Any work that involves the roof must be completed by Benton Roofing.

17. o Proposed substitution will have no adverse effect on other trades and will not affect or delay progress schedule.

o Proposed substitution does not affect dimensions and functional clearances.

• Sprinkler questions:

o Possible to obtain a recent fire pump test from the VA for this building with a key plan showing the location of same.

Any existing drawings showing the existing fire protection standpipes (and area of renovation)

Will be provided at a later date.

18. I see in the specifications HSI is called out in the Basis of Design for the Stickman Utility Module. Upon review of the Headwall drawing elevation with the representative, we do not know if the Stickman Utility Module would lend itself to match the elevation drawing provided as this product does NOT have removable panels. In my discussion with HIS, they would have either the “Compass” or “Array” systems which do have removable panels. There is a price difference in these three products and they have used both of the “Compass” or “Array” systems in other facilities

Headwall provided by the VA and installed by Contractor. Contractor is to

19. The only reference I can find to any materials which may need abated is Lead based paint.

It is referenced in 01 74 19-2 (page 143) and 09 91 00-5 (Page 331) of the specifications they refer you to 02 83 33.13 which is not enclosed in the manual.

Please identify where the materials are (drawing page or spec section) so a quote for the testing and abatement of these material can be obtained from properly licensed contractors.

Also has a phase 1 report been completed? If so forward the phase 1 report with the test results so we are able to obtain quotes

Asbestos report has been included in the Contract Documents.

20. In Section 2.7 BID SUBMISSION of the Solicitation on pages 7-9 it lists all required submission documents, however, on page 43 of the Solicitation it states that there is an attachment called “Past Performance Questionnaire.” Nowhere in the Solicitation does it mention that a PPQ is to be submitted with our bid. Please confirm that offerors are not required to have past performance questionnaires filled out and submitted with the bid packages.

PPQs are NOT required.

21. If the PPQs are to be submitted with our bid, please provide further instruction on what types of jobs should be submitted, how many PPQ’s need to be submitted, and can CPARS Evaluations be submitted in lieu of PPQs.

Please confirm work will be able to be performed during normal business hours with special considerations to noise and vibrations, and that night, weekend and holiday work are only necessary to be scheduled when an interruption in service to the medical center needs to be planned.

The Salisbury VA is a 24-7 active medical facility. Patient quiet hours are 7pm-8am. Weekend and holiday work may be required to meet schedule.

24. Page 01 00 00 -1 of the Specs states that “Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for renovations to Operating Rooms on 4th and 5th Floors of Building 2 as required by drawings and specifications.” Is this the correct description of the project? There is not any OR rooms on this project and all work appears to be taking place on the 5th floor and Roof.

Regarding this project the is no work associated with the OR nor ongoing work on the south wings.

25. Operating rooms will be used during demo and construction. VA OR work hours are 7:30 am to 5:00 pm M-F. Quiet hours follow. Contractor to schedule work accordingly. Weekend and holiday work is assumed.

Weekend and Holiday worked to be scheduled and approved with VA COR. Noise and vibration will be closely monitored and tested.” Please confirm this does not apply to this project.

26. Can you please provide the previously completed abatement report to the bidders, if one is available?

There is an asbestos report include within the Contract Documents.

27. Prior to the renovation of the area, we will conduct a survey to obtain actual quantities for materials that must be abated. For basis of bid, however, how would you like offerors to quantify areas limited by the physical constraints imposed by occupancy of the buildings and accessibility to ACM?

There is an asbestos report include within the Contract Documents.

Most of these materials would still have been Buy American compliant as the tariffs would increase pricing on all steel or wood materials, regardless of origin.
