598-18-105 Correct Electrical Deficiencies, Central Arkansas Veterans Healthcare System The following are a summary of changes for the amendment:

Page 1 of 324 - Changed Last Revised date from “6/20/2018” to “7/26/2018.”

Page 2 of 324 - Added to the TABLE OF CONTENTS:

Appendix E – CAVHS Penetration Permit Policy, Memorandum 138-021 Appendix F – Construction and Renovation Risk Assessment Form Page 5 of 324 - Paragraph 5. Changed the PERIOD OF PERFORMANCE from “270 days” to “360 days.”

Page 56 of 324 - Specification 01 35 26, paragraph 1.8.D - After this paragraph, Insert, “All contractor personnel working on the site for more than 2 days during the contract period shall receive the CAVHS Contractor Safety Training before participating in any work activity. Training will last about one hour and will be coordinated with the CAVHS Safety Office through the COR. All participants shall sign a register that the CAVHS Safety Office will keep on file to document completion of this training requirement.”

Page 71 of 324 - Specification 01 35 26, paragraph 1.18.C.5. After this paragraph insert:

1.19 PENETRATION PERMIT POLICY

A. Contractor shall follow the CAVHS Penetration Permit Policy, Memorandum 138-021, as indicated in Appendix E.

1.20 PRE-CONSTRUCTION RISK ASSESSMENT

A. Contractor shall assist the COR in completing the Construction and Renovation Risk Assessment Form (Appendix F). This assessment is required for the Medical Center to determine what actions will be taken during demolition, construction, or renovation so that hazards to patients, medical center staff, and the general public can be minimized. A separate Risk Assessment shall be required for each separate area of construction, and must be approved by the CAVHS Construction Safety Committee. Contractor shall provide any recommended risk mitigation measures that must be in place prior to the start of work in the area under review.

Page 148 of 324 - Appendix A, (NLR Site Plan). Replace this page in its entirety with the attached page labeled, “North Little Rock Veterans Center Site Plan”. Contractor Onsite Storage has been added.
