Proposals shall be submitted and received by 5:00 p.m. MST November 15, 2018.
A copy of the questions received to date with their answers are also incorporated into this amendment.

The following questions regarding this solicitaiton were received before November 1, 2018. The answers are provided herein.
1. Questions are due this Thursday, and proposals by next Thursday?
Questions were originally due November 1, 2018 and proposals by November 8, 2018. However, the VA has decided to extend the dates. Questions are now due by November 6, 2018 and proposals are due by 5:00pm MST on November 15, 2018.
2. Can you please tell me if this is a new requirement? If not, who is the incumbent, and what is the contract number?
This is a solicitation for follow-on contracts to previously established program.
Current contractors are: American Renal Associates, LLC.; Atlantis Healthcare Group of Puerto Rico, Inc.; DaVita Healthcare Partners Inc.; Deer's Head Center Maryland Department of Health & Mental Hygiene; Dialysis Clinic, Inc.; Dialysis Corp of America, Dialysis Newco, Inc.; Emory Dialysis, LLC; Fresenius Medical Care Holdings Inc.; Independent Dialysis Foundation, Inc; Liberty Dialysis-Hawaii LLC; Liberty Dialysis-North Hawaii LLC; Liberty Dialysis LLC; Physician's Choice Management, LLC; Satellite Healthcare, Inc.; South Fulton Dialysis Center LLC; Trinity Dialysis Clinic Inc.; U.S. Renal Care, Inc.; Wake Forest University Health Sciences; Renal Centers of Guam, LLC; RSA-Guam, LLC; & RSA-Tumon LLC.

3. What is the response date once final RFP issued? Is there a Q/A period?
The final RFP was issued October 29, 2018. Questions are due by November 6, and proposals are due by 5:00 pm MST on November 15, 2018.
4. Is a technical proposal not needed on this – just the signed attestation, facility list, subcontracting plan, pricing, etc.?
The requirements are outlined in solicitation paragraph "E.2 Submission and Contact Information". Regarding Volume 2, Technical, the only required documents are Attachment 3, Attestation Statement, and a completed Attachment 2, Facility List. Volume 3 shall contain price information, past performance information, and small business subcontracting plan (if applicable).
5. Attachment 1, For the 87 page Solicitation/Contract, should we be completing anything? Or will this document be completed by the VA upon award?
Paragraph E.2 "Submission and Contact Information" outlines the requirements for the proposal submissions. Additionally, FAR 52.212-1 and 52.212-3 require actions by the offeror. Note Attachment 1 in this question is not referring to the Quality Assurance Surveillance Plan (QASP), but the solicitation itself as identified on the FBO main page.
6. Attachment 2, For the Quality Assurance Surveillance plan, should we just add the Contractor Representatives under # 3 and sign on page 8?
The solicitation instructions did not outline any required action on the Quality Assurance Surveillance Plan (Contract Attachment 1). Note: Attachment 2 in this question is referring to the Quality Assurance Surveillance Plan (QASP) as it was uploaded as the second document on FBO.gov.
7. Attachment 3, Attestation, should we just complete this for all dialysis facilities under our master company?
Only one Attestation Statement, Attachment 3, is required per proposal assuming it is true for all facilities included in Attachment 2.
8. Attachment 5, Subcontracting plan. How do you define subcontracting? If we do not subcontract, do we need to complete this?
Subcontracting plans are only required for large businesses. A subcontract means any contract entered into by a subcontractor to furnish supplies or services for performance of a prime contract or a subcontract, as defined at FAR 44. It includes but is not limited to purchase orders, and changes and modifications to purchase orders. If large business offeror does not submit a subcontracting plan in Volume 3, they must explain why no small business subcontracting opportunities exist.
9. Attachment 6, do we complete the past performance questionnaire, or does the VA complete this?
As stated in Attachment 6, "This past performance questionnaire is for offerors to send to their current and/or previous references with instructions to return the completed questionnaire to the Government."
10. Attachment 7, is there anything we need to do with this?
No action is needed on Attachment 7, Dialysis Data for Fiscal Year 17 by VISN, it is for informational purposes only.
11. When is the deadline for VA Dialysis Contract, Solicitation 36C79118R0022?
Proposals are due by 5:00 pm MST on November 15, 2018.
12. VA has had time to extend the existing contract while working to come up with this new proposal and we’re given a week? How can VA expect providers to fully review this agreement and have it completed and returned by November 8th?
Thank you for your input and concern. The VA issued the draft solicitation on October 25th. The final solicitation was posted on October 29th. Proposals were originally due November 8th, 2018, but the VA has decided to extend this deadline by one week. Proposals are now due November 15th, 2018 at 5:00 p.m. MST.
13. Is there an extension on questions or submission of the RFP? I am asking because the final solicitation was posted on 10/30 and that seems rather short to review.

14. Both the draft RFP and the final RFP states the due date of proposals is November 8, 2018 by 5:00pm MST. Will you please confirm?
Proposals were due by November 8, 2018 at 5:00 MST, however this amendment extends the deadline to November 15, 2018.
15. When going through the new RFP versus the RFP from 2012, I noticed that the "quality control plan" is not listed as a factor under technical qualifications. Am I missing something or is this year's RFP not calling for a quality control plan?
The requirements are outlined in solicitation paragraph "E.2 Submission and Contact Information". Regarding Volume 2, Technical, the only required documents are Attachment 3, Attestation Statement, and a completed Attachment 2, Facility List. No “quality control plan” is required.
