Justification for Single Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

1. Contracting Activity: Department of Veterans Affairs, VISN 15, National CMOP

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The glucose test strips requested are unique and brand specific for the glucose readers issued by The Department of Veterans Affairs. Using a “generic” or any other test strip with these devices can lead to false readings or no readings at all. Therefore, the diabetic could inadvertently inject to much or too little insulin to treat their diabetes. Not using the specific strips designed for the VA issued device could lead to serious health consequences or even death.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The test strips requested are specifically designed to be read by the specific glucose test meter issued by the Department of Veterans Affairs. Any variance of test strips could lead to a false reading and the Department of Veterans Affairs would be solely responsible for placing the patient in a danger of receiving false readings from their glucose measuring device.
4. Description of market research conducted and results or statement why it was not conducted:
The manufacturer will NOT guarantee accurate glucose test readings using any other test strip than the ones specifically designed and produced for that meter. All Department of Veterans Affairs physicians are aware of the risk of using a third-party test strip and specifically prescribe the correct strips for use. This is a distinctive patient care issue that directly effects the patient’s quality of life.
An incorrect reading has serious consequences for the veteran and those around them.
