B.4 STATEMENT OF WORK

Electric Steam Griddle Fort Meade VA Medical Center

The Fort Meade VAMC is looking to procure a Brand Name or Equal Item to the AccuTemp Model EGF2083A3650-S2 with the following salient characteristics.

· Includes stand with casters

· Electric, 36” X 30” griddle area, 7 ga stainless steel cooking surface

· Digital thermostat & controls

· 4” grease trough, 14 ga stainless steel cabinet

· 208v/60/3-ph, 14.25kW, 40 amps, cord with NEMA 15-50P, cULus, UL EPH

· Provides 200-400 degree F operating temperature range and uniform griddle surface temperatures of plus or minus 5 degree F

· 1-year parts & labor warranty, standard

· Control guard

· Easy front service access to controls

· Preset temperatures capable

· Manager mode to lock out temperature controls

· Over temperature indicator light

· ENERGY STAR compliant

· Hermetically sealed chamber

1. General Conditions

1.1 Delivery Schedule

Items shall be delivered no earlier than the estimated delivery start date and no later than the delivery end date listed in Table below.

1.2 Schedule and Order Management

1.2.1 The awardees actual delivery dates will be confirmed by VA upon award.

1.2.2 Post-award the vendor will coordinate delivery prior to beginning any work (see section 1.3). Once the date has been confirmed, modifications to the schedule are subject to written approval by the VA POC. Schedule changes that are beyond the dates specified in the contract will require approval by the Contracting Officer and a contract modification.

1.3 Delivery and Receiving

Delivery and receipt of the proposed items is anticipated to be directed to the following address:

Confirmation of delivery location will be provided by the VA POC post-award. Delivery must be coordinated with VA NFS POC Ashley Horning (Office): 605-720-7396 for any attempts at delivery. Any item that has not been coordinated will be turned away and any additional charges will be the responsibility of the sender/transportation company. Vendor is responsible for conducting a site visit to verify logistics and loading dock access/egress. Delivery and freight on board (FOB) – Destination.

1.3.1 The vendor is required to define the need for a staging area to accommodate item delivery if needed. Post-award, this information will be confirmed with the vendor.

1.3.2 The delivery of items identified in this document shall take place during normal business hours which are defined as: 0730 to 1630 MST, Monday through Friday, and excluding Federal Holidays.

1.3.3 Delivery trucks will not be permitted to remain at the loading dock. Trucks shall be unloaded, moved from the dock, and then brought back to the dock if required to haul out any waste, tools, or excess materials.

1.3.4 Labeling of delivered items shall include the awarded contract number and VA purchase order number for identification and reference upon receipt of delivery.

1.3.5 Delivery and Installation includes:

Offloading the equipment from the truck, moving the equipment to the correct delivery location in the space, uncrating or unboxing the equipment and removing all trash, setting up the equipment according to the manufacturers specification and testing the equipment to verify that it is operational. Equipment installation paperwork & warranty information must be handed to the approved government contact onsite. See Section 1.6 below.

1.4 Use of Warehouse-Not applicable direct delivery to the new facility

1.4.1 If the vendor requires the use of the Government’s activation contractor’s warehouse to meet the requirements of this contract the vendor must provide a certificate of insurance prior to the delivery and offloading of the item(s). This insurance certificate must be completed and presented to the activation contractor prior to delivery.

1.4.2 The vendor shall communicate through the VA POC any required coordination requirements.

1.5 Clean Up and Disposal

1.5.1 The facility will be responsible for removal of existing steamer and place on dock for removal by facility. The facility will be responsible for the disposing of existing equipment. The facility will dispose of any packaging debris.

1.6 Standard Warranty

1.6.1 The products shall be covered by vendor’s standard warranty terms and provisions. The warranty coverage shall be no less than 1 YEAR per VA policy.

1.6.2 The warranty period set forth therein shall be for one year following the delivery of steam griddle at VABHHCS – Fort Meade.

1.6.3 The vendor shall extend such warranty directly to the customer as if such customer had purchased the Products directly from the vendor.

1.6.4 The vendor shall warrant the products against defects in material and workmanship under normal use and service, repair or replace at vendor’s cost any defective Product (or issue a credit or refund based on the purchase price paid therefor); and

1.6.5 The vendor shall pay (or refund the amount of) all freight and shipping charges for any defective Products returned under this paragraph.
