If no loaded elsewhere (other than the TPOC) (i.e., patient refuses transport) the Flat Rate charges will apply. | |
| c. | WAITING TIME: Rates to include one quarter hour of waiting time at either end or both ends of a trip. The Contractor will be reimbursed at the rate indicated per quarter hour, in excess of the initial one quarter hour allowed. Anytime that the Contractor anticipates a delay for which he expects to claim reimbursement, the Contractor must call the Contracting Officer’s Representative (COR) and advise that a delay is anticipated and the approximate length of the delay for which he expects to claim reimbursement. This call is only for the purpose of verifying his arrival time at the pick-up or drop-off point and is not necessary if the Contractor anticipates no delay for which he will claim reimbursement. However, if prior notification has been given to the Contractor that waiting time in excess of the initial 15 minutes will be required, the Contractor is not obligated to call for verification. The Contractor will be responsible for all costs incurred in verifying waiting time. | |
| d. | For the purpose of this contract, any waivers or variances granted to the Contractor by the EMSRB (Minnesota Rule 4690.8100) must be approved by the Contracting Officer. | |
| 5. | DRIVERS AND ATTENDANTS: | |
| a. | The prices quoted in the Rate Table include services of a qualified driver and for medical personnel (attendants) as appropriate for emergency care vehicle on any assignment where a patient is transported in the performance of this contract. | |
| b. | Drivers and attendants must possess a current emergency care course certificate authorized by rules adopted by the board according to chapter 144E; and receive training approved by the licensee’s medical director. Annually, the medical director or the medical director's designee shall assess the practical skills of each person on the ambulance service roster and sign a statement verifying the proficiency of each person. The statements shall be maintained in the licensee's files. | |
| c. | Record of each employee as to training, character and physical capabilities of performing the duties of an ambulance driver or attendant must be maintained and made available for inspection upon request by the Contracting Officer or her designee. Proof in the form of current certificates that appropriate training has been successfully completed must be available upon request. | |
| d. | All drivers and medical personnel must be certified, licensed or otherwise recognized by the local, state, or regional government or public entity where the ambulance service is operated or by which it is governed. |
| e. | All drivers and medical personnel must be enrolled periodically in "refresher" continuing education, or advanced training programs as required by the local or state government entity but in no instance shall this be less frequently than every two years. Such refresher training must be available to the Contracting Officer or her designee for inspection and approval. | |

| a. | The ambulances provided under the terms of this contract must meet the standards for vehicle and equipment established by the EMSRB (to include inspections of facility, vehicle, employee education records, and vehicle maintenance). The government will consider inspections as conducted by EMSRB. However, the government does not waive its right to conduct inspections on either a scheduled or unscheduled basis (see paragraph 11 below). | |
| b. | The Contractor must have an adequate number of vehicles to meet the requirements of this solicitation and resulting contract. | |
| c. | Every ambulance offering service shall be equipped as required by the board and carry at least the minimal equipment necessary for the type of service to be provided, as determined by standards adopted by the board. | |
| d. | Vehicles and equipment used in the performance of this contract must be clean and orderly and kept in excellent operating conditions at all times. | |
| e. | Maintenance of vehicle and equipment must be on a periodic basis and meet or exceed manufacturer's recommendations; documentation of maintenance must be kept on file at owner's premises or in the vehicle. | |
| f. | Records of such maintenance must be made available for inspection to the Contracting Officer or her designee upon request. | |
| g. | Smoking in any portion of the ambulance is prohibited. | |
| 7. | PATIENT TRANSPORTATION: | |
| a. | Number of Patients: It is understood and agreed that only one (1) patient will be transported on a trip unless specifically authorized by the TPOC. When more than one (1) patient is transported on a (BLS) trip, Contractor may invoice mileage for the longest distance over which a single patient is transported plus the base rate for the first transported patient and the trip rate for the second patient (see Miscellaneous Supplies & Services section in schedule of items). Contractor must ensure that the pick-ups and drop-offs are scheduled so that the total distance traveled will result in the most economical charge to the Government. | |
| b. | Contractor shall not release the patient from their custody, unless custody is released to some responsible party. Contractor shall take every reasonable measure to assure the well being of the patient at their destination. Patients brought in by ambulance for a scheduled clinic appointment shall be delivered directly to that specific clinic, released to the custody of clinic personnel, and picked up in the clinic after the appointment by the ambulance crew. | |
| c. | Contractor is required to notify the Contracting Officer or designee of any unusual events including, but not limited to, accidents and safety problems that occur that involve VA beneficiaries being transported under this contract. This notification will be made within one (1) hour by telephone and, if requested, a written report of the event will be delivered to the Contracting Officer or her designee by close of business the next working day. Failure to comply with this provision may be grounds for default. | |
| d. | An authorized official of the medical center may, in the best interest of the patient, allow one (1) relative or concerned other to accompany him or her at no additional cost to the Government when accompaniment will not interfere with care of patient. | |
| 8. | HOURS OF SERVICE: |
Contractor must provide service Monday through Friday (excluding holidays) for TPOC for the term of this contract. The majority of the requests for service for TPOC will be between the hours of 6:00 a.m. and 6:00 p.m., Monday through Friday. The Contractor must provide TPOC with a method of contacting him/her, 24-hours per day.
9. REQUEST FOR SERVICES:
a. Request for services will be made in writing (facsmile, e-mail, mail) or by telephone by an authorized representative of the VA. When ordering services the VA shall provide the Contractor with the following type of information:
· Mode of transportation required;
· Required date and time of arrival;
· Name of Beneficiary
· Pick-up and delivery point;
· Type and number of additional medical care specialists required;
· Any other special instructions;
| b. Generally services provided under this agreement shall be scheduled in advance with the Contractor for non-emergency ambulance transportation, schedule requirements are defined as follows: | |||
| 1. | Scheduled Trips refers to those trips for which the Contractor has been given advance notice defined as notice given by 4:30 p.m. the previous day of pick up for required services and a specific pick-up time. These transports will always be on time for patient pick-up. | ||
| 2. | Unscheduled Trips are those trips required on an as needed basis and advance notice is not given. Response time for ambulance’s arrival at destination is within one and one half (1 ½) hour after notification of need. | ||
| 3. | Service scheduled from the Twin Ports Outpatient Clinic generally will be requested for same day service and typically at a higher level of care (i.e. ALS), with a one and one half (1 1/2) hour response time for ambulance’s arrival at destination. | ||
| c. | Contractor must notify the Contracting Officer’s Representative (COR) for VAMC/TPOPC at least eight (8) hours in advance of need when Contractor believes he/she may be unable to meet the number of pre-scheduled trips required. |
d. In the case of inclement weather, the decision as to whether or not a VA beneficiary will be transported will be discussed with the Contractor; however, the final decision as the whether a patient transport will be ordered or cancelled remains with the VA.
e. The contractor shall, at all times during the contract period, have on his/her premises a functioning fax machine to communicate requirements with the VA (i.e., requests for service, patient incidents, special correspondence, etc.). Each contractor shall indicate if there is e-mail availability for their company to complete communications with the VA (use of this communication system will be mutually agreed upon after award of contract before any communications are completed in this manner). If the contractor’s place of business is located beyond the local telephone call zone of the VA (area code 612), the contractor shall install and provide to the VA, a toll free number for accepting the VA calls. It is estimated that 90% of daily contacts regarding questions and/or problems are initially addressed by telephone.
| 10. | RESPONSE TIME: |
| a. | Twin Ports: If the Contractor fails to dispatch ambulance service to arrive at the destination within one and one half (1 1/2) hours after receiving a request for a non-emergent order, the VAMC reserves the right to obtain the service from another source and to charge the Contractor with any excess cost which may result therein (VA will take into consideration situations such as inclement weather or unusual traffic congestion). In the event that no other vendor is available, a deduct in the amount of ten percent (10%) of the flat rate charge for the appropriate type service (BLS, ALS,) may be accessed per every quarter hour (15 minutes) in excess of the allowable response time (1 1/2 hours). The VA will be the sole judge in determining when to order service from another source. |
b. Ground Ambulance Response to Air Ambulance Ground Pick Up Transport Services: VA will provide ground ambulance contractor with an estimated time of arrival in accordance with paragraphs 9b (1) & (2) when services are required for pick-up of patients transported to the Minneapolis VAMC through air ambulance. While in flight, the VA air ambulance contractor will notify the ground ambulance contractor with an updated estimated time of flight arrival, normally one (1) hour prior to flights actual arrival (dependant upon departure location). In the event that contractor fails to pick up air ambulance patient within fifteen (15) minutes of scheduled landing, the VAMC reserves the right to bill ground ambulance contractor established air crew layover fee in fifteen minute increments.
c. Response Time Compliance Rate: The contractor shall maintain at a minimum an 80% compliance rate on responses to pick-up or deliver during the contract year.
