Antimicrobial Clinical Surveillance Software for Infection Prevention (e.g. COVID-19) and Antimicrobial Stewardship Program
VISN 23 facilities need an automated clinical decision support tool for Infection Prevention and Antimicrobial Stewardship Programs. In addition to Healthcare-Associated Infection, influenza and COVID-19 surveillance for Infection Prevention the system should also include antimicrobial stewardship program surveillance. Each facility requires a unique instance of the software to interface with each facility’s legacy electronic health record (CPRS).
This system will enable the VA real time reporting of healthcare related events and information regarding patients. Information will be pulled in real time from Vista, including but not limited to ADT, LAB, Microbiology, Surgery, Inpatient Pharmacy, Radiology, Bar Code Medication Administration, Vital Signs, Problem List and Outpatient Pharmacy. Using a browser-based access platform it will allow for access to information in real time at each facility. Other modules allow for tracking of clinical alerts, adverse drug events, patient flags, infection prevention, multi-drug resistant organism management and data reports/mining.
This will allow for an increase in patient safety and care, surveillance of resistant organisms and outbreaks within the hospital and improve the antimicrobial stewardship at each facility. Having a more robust system in place to promote antimicrobial stewardship will minimize inappropriate antibiotic selection, maximize appropriate selection of antibiotics, decrease antimicrobial costs, allow monitoring and enhancement of antimicrobial prescription, and better utilization of facility specific antibiograms. This system will also allow the VA to meet regulatory requirements set by Joint Commission Standards, National Patient Safety Goals as part of the Joint Commission Requirements, and VHA Directives.
DSS System Infectious Disease Analysis & Monitoring Software – Brand Name or Equal The following Antimicrobial Clinical Surveillance Software for Infection Prevention (e.g. COVID-19) and Antimicrobial Stewardship Program to be added to each VISN 23 facility shall meet the following requirements for operation:
Requirement
Description

The system must be able to interface with VistA in real time, for surveillance, reporting and alerting.
TRM Approved
The system and all its parts must be approved under the Technical Reference Model.
Web based application
The interface must be able to be accessed through a web browser for easier access across the Minneapolis VA.
Unlimited users and user groups at no additional charge
Ability to be used by all inpatient and outpatient VA providers, with unlimited provider and group ability.
Ability to pull relevant information
The system must be able to pull VistA information from the following systems in real time:
· ADT
· LAB
· Microbiology
· Surgery
· Inpatient Pharmacy
· Radiology
· Bar Code Medication Administration
· Vital Signs
· Problem List
· Outpatient Pharmacy
Includes Necessary Modules
For detailed module requirements see appendix below

· Ability to send alerts through email
· Ability to flag patients with different identifiers
· Reporting function with the ability to customize based on user defined parameters (e.g. pharmacy, lab, microbiology)
Virtual Server Compatibility
All servers included within this system must be able to be run as a virtual server as long as the minimum specs for hosting machine are met. This includes a VISN-wide shared system.

· Adverse drug event prevention: Identify potential problems in the medication-use process, including therapeutic products with Expanded Use Authorization (EUA) for COVID-19.
· Anticoagulation management: Meet the National Patient Safety Goal for the reduction of patient harm associated with therapy, efficiently review anticoagulant orders, relevant lab data and alerts and monitor achievement of therapeutic goals.
· Days of Therapy (DOT) tracking: Track and report antimicrobial use with the preferred metric of the CDC.
· Timely alerts: Within seconds of a potential issue being reported, identify and prioritize patients for intervention using a wide range of critical, targeted clinical parameters.
· Rounds Assistant: Rounds Assistant is an end-user tool to create custom templates that are then auto populated in the system.
· Infection Control Assistant: System’s infection control module assists with surveillance, documentation of infections, as well as strengthens antibiotic stewardship practices.

(b) All delivery orders or task orders are subject to the terms and conditions of this contract. In the event of conflict between a delivery order or task order and this contract, the contract shall control.
(c) A delivery order or task order is considered "issued" when—
(1) If sent by mail (includes transmittal by U.S. mail or private delivery service), the Government deposits the order in the mail;
(2) If sent by fax, the Government transmits the order to the Contractor’s fax number; or
(3) If sent electronically, the Government either—

(b) Maximum order. The Contractor is not obligated to honor—
(1) Any order for a single item in excess of $1,000,000.00;
(2) Any order for a combination of items in excess of $3,000,000.00; or
(3) A series of orders from the same ordering office within 30 calendar days that together call for quantities exceeding the limitation in paragraph (b)(1) or (2) of this section.
(c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section.
(d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 30 calendar days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source.
(End of Clause)
C.4 52.216-21 REQUIREMENTS (OCT 1995)
(a) This is a requirements contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies or services specified in the Schedule are estimates only and are not purchased by this contract. Except as this contract may otherwise provide, if the Government's requirements do not result in orders in the quantities described as "estimated" or "maximum" in the Schedule, that fact shall not constitute the basis for an equitable price adjustment.
(b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. Subject to any limitations in the Order Limitations clause or elsewhere in this contract, the Contractor shall furnish to the Government all supplies or services specified in the Schedule and called for by orders issued in accordance with the Ordering clause. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations.
(c) Except as this contract otherwise provides, the Government shall order from the Contractor all the supplies or services specified in the Schedule that are required to be purchased by the Government activity or activities specified in the Schedule.
(d) The Government is not required to purchase from the Contractor requirements in excess of any limit on total orders under this contract.
(e) If the Government urgently requires delivery of any quantity of an item before the earliest date that delivery may be specified under this contract, and if the Contractor will not accept an order providing for the accelerated delivery, the Government may acquire the urgently required goods or services from another source.
(f) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after the end of the effective period.
(End of Clause)

(a) The Government may extend the term of this contract by written notice to the Contractor within 15 calendar days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 calendar days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) year.
(End of Clause) C.6 VAAR 852.212-70 PROVISIONS AND CLAUSES APPLICABLE TO VA ACQUISITION OF COMMERCIAL ITEMS (APR 2020)
