Statement of Work

Scope of Work The purpose of this contract is to ensure that appropriate regular cleaning is performed to ensure compliance to Joint Commission regarding Ice Machines in a medical setting. These units support direct patient care and are required for medical recovery. Due to the current workload and the detailed maintenance required to comply with the strict standards of sanitation, a service contract with a contractor is justified to perform the work. Maintenance as stated in the owner’s manual will be required on a quarterly basis.

Requirements:

The contractor is required to supply all materials and labor to complete the objectives specifically stated in the scope of work. Work will occur in patient care areas during normal business hours (8:00am-4:30pm); therefore, work must be scheduled two business days in advance with the VA contracting officers’ technical representative. Services will be rendered quarterly and will be limited to appropriate completion within the selected calendar month. Payment will be made quarterly in arrears. Professional conduct and appropriate dress is required inside the premise. The contractor is required to report to Engineering Room BD-50 to check out a contractor’s identification badge. This badge must be returned to Engineering following the completion of work.

Instructions:

The contractor must perform the equipment manufactures exact cleaning procedures quarterly for each piece of equipment. Any piece of equipment will not be allowed to be out of service for more than one singular business day for cleaning. If repairs are required during the cleaning process, this will be coordinated through Engineering. Standard cleaning services as defined by the equipment manufacturer must be provided in the months of July, October, January and April of each fiscal PERIOD. Documentation of service must be provided in the form of a label adhered to each piece of equipment that indicates the initials of the service and date of service (MM/YY). Additional documentation will consist of an invoice for services indicating specific dates provided to the VA to ensure prompt payment.
