About this file
This document is a Past Performance Questionnaire used to evaluate contractor performance on completed federal contracts. The form captures comprehensive information about a contractor's past work, including the contractor name, role, contract number, contract type (Firm Fixed Price, T&M/LH, Cost Reimbursement, or other), procuring agency details, period of performance, dollar amounts, and a description of the contract scope. Respondents must provide ratings and mandatory comments across seven evaluation factors: Quality of Work, Personnel, Cost/Budget Control, Customer Satisfaction, Timeliness of Performance/Deliverables, Business Relations, and Overall Performance.
The questionnaire requires evaluators to assign adjectival ratings of Unsatisfactory, Marginal, Satisfactory, Very Good, or Exceptional for each factor, supported by detailed justification in the comments section. A final section asks whether the evaluator would select the contractor again, with options for "Definitely," "Probably," or "Definitely Not." The document includes comprehensive rating guidelines defining performance expectations for each evaluation factor and rating level, addressing compliance with contract requirements, cost management, delivery schedules, responsiveness, and business conduct. The form concludes with signature blocks for the evaluator's name, title, and date.
