QUESTIONS AND ANSWERS

Question 1: What specific kind of air monitoring will you all be requesting?

Answer 1: General indoor air quality, testing for carbon dioxide, carbon monoxide, airborne particulates, allergenic particles, miscellaneous indoor gases (sulfur dioxide, nitrogen dioxide, hydrogen sulfide) and Volatile Organic Compounds.

Question 2: Will it be general air quality monitoring or asbestos monitoring?

Answer 2: General, not Asbestos.

Question 3: How long is the project expected to take?

Answer 3: This will be annual testing/monitoring. Actual length of time to complete depends on many factors outside VA control.

Question 4: How would you like the proposal submitted?

Answer 4: Submit via email to Rochelle.Bennett@va.gov.

Question 5: What is the frequency of airborne testing?

Answer 5: Once annually.

Question 6: What is the number of samples that need to be collected?

Answer 6: A representative number of samples, enough to have a reasonable idea of the overall air quality within the facility.

Question 7: What is the number of areas that need to be sampled?

Answer 7: A representative number of samples, enough to have a reasonable idea of the overall air quality within the facility.

Question 8: In how many areas does formaldehyde need to be measured?

Answer 8: All areas.

Question 9: Will we be able to put in a ‘partial bidding’ for this project?

Answer 9: No.

Question 10: Could you provide a specific definition of what you mean by "indoor air quality" testing in your contract? Are you referring to air sampling for airborne spores and particulates, or do you require testing for other types of contaminants or chemicals?

Answer 10: See Question 1.

Question 11: Page 28 of 47, Section E.1 General Instructions for Quotation Submission subparagraph Submittal Instructions bullet a. Offeror must possess a 30-hour Occupational Safety and Health Administration (OSHA) safety card. This is an individually held certif ication.

Where within our Quote should this card be provided and for whom?

Answer 11: 30 Hr. OSHA should be held by the superintendent, a supervisor, or foreman etc.

The certif icate is normally included as part of the quote package.

Question 12: Is the Contractor to provide a cost for the Over and Above Repair portion of the SOW and Cost Schedule as part of this Request for Quote?

Answer 12: No.

Question 13: Factor 3 Price – Other than the Section B and the Pricing Sheets for the CLINS provided on pages 5-7 of 47, should we provide additional forms, such as the SF1449, Representations and Certif ications, etc., within this Factor?

Answer 13: At your discretion.

Question 14: Would government please provide the number of floors (including basements) for each building?

Answer 14: Main Hospital has 4 floors with 4 interstitial spaces, and a tunnel complex underground.

Outpatient pharmacy-2 floors Valor building-2 floors Cancer center- 2 floors All other buildings are single story.

Question 15: HVAC Inspections: What (if any) above ceiling access protocols are required for the buildings in this solicitation? Does access to the above ceiling spaces require mini containments? Mobile above ceiling access units with integrated HEPA filtration? Or will the above ceiling access controls be minimal (that is, no containment required).

Answer 15: No containment required for inspections. If actual work is to be performed (drilling, cutting, modifying ductwork for example) then containment may need to be required and will be determined on a case-by-case basis depending on sensitivity of the area, amount, nature, and duration of work.

Question 16: HVAC Inspections: If above ceiling access protocols are required for some but not all buildings, then what buildings require above ceiling access protocols?

Answer 16: See Question 15.

Question 17: HVAC Inspections: Is a CSLB C-20 license required for the HVAC inspections?

Answer 17: Yes.

Question 18: HVAC Inspections: Item (IV)(A)(1), Visual inspection. What parameters are to be included in the visual assessment of these components? Is it mechanical operation and performance of equipment? Or microbial/dust related?

Answer 18: Both.

Question 19: HVAC Inspections: Access points to ducting.

19a. Presumably these services have been performed at the VA Loma Linda (given past contracts). Are access points already available at these locations in all buildings (that is at the "main ducts leaving each AHU...)?

Answer 19a: Not all areas will have access points.

19b. Or will access points be required to be installed in all buildings at all AHUs?

Answer 19g: In some, but not all.

Question 20: Air Quality Testing:
