About this file

This document contains questions and answers regarding a federal contract opportunity with the Department of Veterans Affairs to replace carpet in the Neurology Suite at the VA Loma Linda Healthcare System. Approximately 550 square feet of carpet must be replaced in four rooms, using Achromatic Carpet Tile in black to match the existing hospital standard. A contractor with a C-15 license is eligible to bid. The awarded contractor must follow the requirements outlined in the Infection Control Risk Assessment permit, including maintaining necessary containment until completion. While only a superintendent is explicitly required, management roles may be combined. Furniture will need to be moved either to the hallway or adjoining rooms with vinyl flooring. A floorplan is provided highlighting the rooms requiring service. The agency prefers bids remain valid for one month. Funds for additional unforeseen work will be addressed through modifications to the initial zero-dollar contract line item as necessary.

Q: Please confirm in addition to the Superintendent, the awarded prime contractor must also have (1) one full time Quality Control Manager and (1) one full time Site Safety Health Officer at all times while work is taking place making this three full time individuals required by the VA.

A: I am not aware of the requirement for the contractor to provide (1) one full time Quality Control

Manager and (1) one full time Site Safety Health Officer. The hospital has its own Construction Safety

Committee. They will go over a risk assessment and provide Infectious Control Risk Assessment (ICRA) permit that is to be posted at the entrance of the jobsite. On that permit, it will detail all requirements and necessary containment that must be maintained until the job is complete and must be signed off daily. The hospital also has a Safety Department. The jobsite will be inspected by the COR daily, and by the Construction Safety Committee daily.

Q: Can any of the three Management personnel wear additional hats and serve dual or tri roles?

A: That would be up to the contractor. The only requirement that I am aware of is to follow the instructions that’s outlined in the ICRA permit.

Q: How many square feet of carpet will need to be replaced and which type/brand of carpet has been pre-selected.

A: There is approximately 550 square that needs to be replaced in rooms 2A-53A, 2A-54, 2A-57, and 2A-

60. The contractor shall install Achromatic Carpet Tile, 5T107, Color “Black”

Q: Will the VA accept alternates to the Tarkett basis of design or is this required to match existing? I wanted to understand the potential for alternates.

A: The hospital standard for carpet is Achromatic Carpet Tile, 5T107, Color “Black”. No replacements.

Q: I found this solicitation on Dodge Data & Analytics and was wondering if this job can be bid by contractors with a C-15 license?

A: Yes

Q: Will the VA specify how long the offer must remain valid after submittal?

A: A month would be preferred.

Q: If an amount is required for CLIN 0002 – Unforeseen Circumstances, will the VA specify what this allowance amount should be?

A: Leave CLIN 0002 blank. This is an emergency CLIN which allows for additional work to be completed if required. If something additional is needed than originally planned, a quote from the vendor will be submitted, a modification will be created, and CLIN 0002 will be changed from an amount of $0.00 to the amount of the required work. Emergency CLINs

Q: Room 2A-57 has ceramic tile on the wall. With 4” cove base under it. The VA Loma Linda standard for cove base is 6”. Please confirm that 6” base is to be used. If 6” base is used will it be acceptable to notch the base around the wall tile?

A: The standard for cove base is 6 inches. In room 2A-57, there is about 5 linear feet that will need 4 inch cove base. The contractor can install 6 inch cove base, and in the area where the tile is, that section can be cut down to 4 inch to fit underneath the wall tile.

Q: Are there any pieces of furniture or other obstructions that have to be moved and where can they be moved to?

A: There is furniture that will have to be moved. The furniture can be moved into either the hallway just outside the Neurology Suite, or the adjoining rooms that are in the suite that has vinyl flooring, until the project is complete.

Q: Are there floorplans/drawings?

A: Please see attachment “Neurology Suite Drawing”. The rooms that need service are highlighted.
