STATEMENT OF WORK

1. GENERAL SCOPE

The Veterans Health Administration (VHA) has a requirement for Platform Beds, Riser, and Mattress for the Long Beach VA Medical Center. This requirement is to replace the existing medical beds on the Geropsychiatry unit to platform beds due to the requirement of new national directive for mental health unit.

This requirement includes healthcare furniture and maintenance/warranty services. The vendor shall provide all resources necessary to accomplish the deliverables in this Statement of Work (SOW) for Platform Beds, Riser, and Mattress. The contractor shall provide all materials, and transportation/ delivery to satisfy the needs of each ordering activity.

2. PERFORMANCE REQUIREMENTS

The contractor shall provide and be responsible for the technical assistance, development, and generation for final specifications.

Furniture delivery 5901 E. 7th Street, Building 149 Receiving Warehouse, Long Beach, CA 90822 and Period of Performance is 90 days after receipt of order. DROP SHIP ONLY.

The Government recognized US holidays are: New Year’s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Juneteenth Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Sunday, it is observed on Monday.

The contractor shall:

2.1. Provide and be responsible for the technical assistance, development, and generation for final specifications.

2.2. Provide the specified product that will meet or exceed the following specifications.

2.3. Coordinate all products to order, ship, and deliver new furniture from the manufacturer until final acceptance by the VA Point of Contact (POC).

2.4. Coordinate product delivery per instructions noted in Section 10: Delivery& Schedule.

2.5. Provide the VA POC with tracking information when it is available to vendor. Call to schedule delivery. A 14-day advanced notice is needed to assure manpower is available to receive items.

2.6. Provide maintenance manuals for all products specified.

3. VENDOR RESPONSIBILITIES

3.1. Upon contract award, preparation of order will be monitored by:

3.1.1. The vendor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by VA POC.

3.1.2. The product must not be placed into production without clearance from the VA POC.

3.2. Order delivery will be monitored by:

3.2.1. The vendor shall track manufacturing schedule and notify VA POC with updates of estimated completion date by email.

3.2.2. All orders shall be delivered Free On-Board (FOB) destination to the delivery address listed.

3.2.3. The vendor shall contact VA POC minimum 14 days in advance to schedule delivery.

3.2.4. In the event there is a delay in delivery, Vendor must notify the VA POC immediately in writing via email.

4. GENERAL SPECIFICATIONS/SALIENT CHARACTERISTICS

4.1. Norix Attenda Standard Platform Bed or Equal-to-Product

Equal-to-Product Salient Characteristics:

· Must have a minimum dimension of 84”L x 40”W x 15.5”H (+ or – 1/8”).

· Must be floor-anchor mounted bed with optional restraint rings.

· Must be a slightly textured, maintenance-free surface that permits easy cleaning.

· Must be rotationally molded.

· Must have an anti-ligature design with no internal openings.

· Must have high-impact polyethylene with ultraviolet light stabilizers.

· Must be chemically resistant to blood, vinegar, urine, feces, salt solution, and chlorine solution.

· Must be filled with rigid polyurethane foam for durability and sound absorption.

· Must meet or exceed the State of California, Technical Bulletin No. 133, Flammability Test Procedure for Seating Furniture for Use in Public Occupancies.

· Must meet or exceed the State of California, Technical Bulletin No. 117, Flammability Test Procedure for Seating Furniture for Use in Public Occupancies.

· Must be UL1056 Fire Test of Upholstered Furniture.

· Must be ASTM E1537 Test Method for Fire Testing of Real Scale Upholstered Furniture Items.

· Must be tested to minimum 500 lbs and maximum 1000 lbs static load for safety.

· Must be GREENGUARD and GREENGUARD Gold Certified by UL Environment. Product certified for low chemical emissions to UL 2818. UL.COM/GG.

· Must come in a sand or similar color (e.g., beige, tan) for unit/ward standardization.

4.2. Norix Attenda Riser or Equal-to-Product

Equal-to-Product Salient Characteristics:

· Must have a minimum dimension of 83.5”L x 39.5”W x 8.5”H (+ or – 1/8”).

· Must be a floor-anchor mounted bed with optional restraint rings.

· Must be slightly textured maintenance free surface allows easy cleaning.

· Must be rotationally molded.

· Must have anti-ligature design with no internal openings.

· Must be made of high-impact polyethylene with ultraviolet light stabilizers.

· Must be chemically resistant to blood, vinegar, urine, feces, salt solution, and chlorine solution.

· Must meet the State of California, Technical Bulletin No. 133, Flammability Test Procedure for Seating Furniture for Use in Public Occupancies.

· Must meet the State of California, Technical Bulletin No. 117, Flammability Test Procedure for Seating Furniture for Use in Public Occupancies.

· Must be UL1056 Fire Test of Upholstered Furniture.

· Must be ASTM E1537 Test Method for Fire Testing of Real Scale Upholstered Furniture Items.

· Must be GREENGUARD and GREENGUARD Gold Certified by UL Environment. Product certified for low chemical emissions to UL 2818. UL.COM/GG.

· Must come in a dove or similar color (e.g., grey) for unit/ward standardization.

4.3. Norix Remedy Blue Sealed Seam Mattress 82”x38” or Equal-to-Product

Equal-to-Product Salient Characteristics:

· Must have a minimum dimension of 82”L x 38”W x 6”H (+ or – 1/8”).

· Must be an inverted RF welded sealed seam.

· Must have a core made of densified polyester.

· Must have a cover material made of polyurethane coated polyester.

· Must be fluid, crack, peel, abrasion and bed bug resistant, anti-microbial, and anti-fungal.

· Must provide a virus barrier and is resistant to delaminating for greater infection control.

· Must have a breathable vent that repels oil, water, blood, urine, and other fluids.

· Must be compliant with flammability standard 16 CFR Part 1633.

· Must be compliant with flammability standard 16 CFR Part 1632.

· Must be compliant with flammability standard California Technical Bulletin 129.

· Must be tested to ASTM F1566 Hexagonal Roller Mattress Durability Test.

· Must be tested to ASTM D751 Hydrostatic Resistance Test for Coated Fabrics.

· Must be GREENGUARD and GREENGUARD Gold Certified by UL Environment. Product certified for low chemical emissions to UL 2818. UL.COM/GG.

· Must come in a blue or similar color (e.g., navy) for unit/ward standardization.

5. QUANTITIES

The quantities of each item are listed as noted below. Awarded vendor shall include purchase order number on all shipping labels to include contents.

6. DESIGN CONTRACT TASKS/REQUIREMENTS

9.1 The contractor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by VA Interior Designer.

9.2 The product must not be placed into production without clearance from the VA POC.

9.3 The vendor shall track manufacturing schedule and notify VA Interior Designer with updates of estimated completion date by email.

9.4 The contractor shall provide 30 days free storage. [30 days standard]

9.5 The Government maintains the option to modify the normal work week, days, and hours, as necessary for the accomplishment of VHA mission.

7. DELIVERY & SCHEDULE

10.1 All furniture shall be tagged with the contract number and purchase order (PO) number.

10.2 Vendor must not leave delivery trucks unattended and must be removed from the dock or other-directed area after unloading.

10.3 Vendor shall contact the VA POC at least 14 business days prior to shipping to ensure that the building is ready to receive product.

10.4 Vendor shall provide the VA POC with scheduled date and time that the delivery truck will be arriving a minimum 24 hours in advance.

10.5 Vendor must coordinate for freight company and/or truck driver to contact VA POC at least 30 minutes prior to scheduled delivery.

10.6 Vendor shall place all orders, track orders, schedule deliveries and coordinate trucks with the manufacturer for manufacturing and delivery according to VA project schedule.

10.7 In the event there is a delay in delivery, the vendor shall notify the assigned VA POC immediately in writing via email.

10.8 Vendor shall ensure that the driver of the delivery vehicle has copies of the Bill Of Materials/ delivery orders/ Packing Slips and shall not attempt to deliver any items before the scheduled delivery date.

10.9 The contractor shall call the POC to schedule delivery with POC at least 14 business days before intended delivery date.

8. INSPECTION AND ACCEPTANCE

11.1 Vendor must provide a Bill of Materials with subtotals and tagged per CLIN.

11.2 Vendor must provide cut sheets with product detail for each line item marked on shipment.

11.3 Products will only be accepted if they are determined to be of acceptable quality and working order, as determined by the VA POC.

11.4 The VA POC can perform inspection of the work performed without the presence of the contractor and determine whether the work performed is within standards expressed in this scope of work.

9. WARRANTY

12.1 All manufacturers must offer a standard warranty that product is free from defects in materials and workmanship (Includes shipping, parts and labor for the repair or replacement of defective items.).

12.2 Vendor must provide 10-year limited replacement warranty for platform beds and risers.

12.3 Vendor must provide 5-year limited replacement warranty for mattresses.
