Statement of Work
ABL90 FLEX PLUS Analyzers Lease
BACKGROUND
VHA Loma Linda Healthcare System currently uses the ABL90 FLEX PLUS analyzer. It is an in vitro diagnostic, portable, automated analyzer that quantitatively measures, pH, blood gases, electrolytes, glucose, lactate and oximetry in heparinized whole blood, and neonatal bilirubin in heparinized capillary whole blood.
SCOPE OF WORK
Contractor shall provide labor, parts, materials, equipment, installation on premises, in-service and user training for the lease of the ABL90 FLEX PLUS analyzer within the VHA Loma Linda Healthcare System.
PERFORMANCE PERIOD
The Period of Performance (POP) shall be a Base Year Period of 12 months with the Governments option to extend the contract for four (4) 12-month option periods. Base Year Coverage shall commence on May 1, 2020.

PLACE OF PERFORMANCE
VHA Loma Linda Healthcare System Attn: Medical Service 11201 Benton St.
Loma Linda, CA 92357
DELIVERY LOCATION, NORMAL WORK HOURS AND INSTRUCTIONS:
All work shall be scheduled during normal work hours (6:00 AM to 6:00 PM) Monday – Friday, Pacific Standard Time (PST), excluding Federal Holidays. Work shall not impact patient care services.
Time of delivery specified or mutually agreed to at the time of receipt of telephone orders shall become mandatory upon the Contractor’s acceptance to commitment.
Delivery of supplies by the scheduled time will be complete except as otherwise authorized by Government personnel.
If the Contractor cannot make delivery and an outside source must be utilized, the Contractor will be responsible for paying the difference in cost.

The President of the United States of America may declare additional days as a Federal holiday, and if so, this holiday shall automatically be added to the list above. When a holiday falls on a Sunday, the following Monday shall be observed as a Federal holiday. When a holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday.
PERFORMANCE REQUIREMENTS
Contractor shall provide labor, parts, materials, equipment, installation on premises, in-service and user training for the lease of the ABL90 FLEX PLUS analyzer within the VHA Loma Linda Healthcare System.
Contractor shall provide the use of the ABL90 FLEX PLUS Analyzers with supplies and reagents sufficient to operate the analyzer on a continuous basis, under normal operating conditions, observing all manufacturers recommendations.
Contractor shall provide a warranty for one year and service coverage for 4 years.
Contractor shall provide quality control material and 0 linearity kits annually for the Analyzers.
Contractor shall provide specification and warranty documentation related to all equipment installed and placed at the VHA Loma Linda Healthcare System.
Contractor shall check-in with Point of Contact (POC) upon arrival at the VHA Loma Linda Healthcare System.
Contractor shall be issued Temporary ID Badges from the VHA Police Department that are always to be worn while on VHA property.
Upon completion Contractor shall contact the POC upon completion of service. A Final Inspection of services shall be documented.
PARKING AND SPEED LIMIT
Park only in areas designated by the facility guidelines or otherwise directed by Police and Security Service personnel. Contractor shall conform to the speed limit set at each Government facility. Should Contractor personnel receive a citation for parking, speeding or other infraction from VA Security, the offenses will be handled in Federal Court as they are Federal offenses.
INVOICE AND PAYMENT
No advanced payments will be authorized. The contractor shall submit invoices on a monthly basis. The invoice must have at a minimum, the contract number, assigned purchase order, assigned obligation number, date order was placed, date order was shipped, and any additional necessary detailed information to identify service rendered. Payment on invoice may be delayed if invoice(s) are not completed properly.
