Amendment to RFP 36C26218R0615 A00002 Page 2 of 2

The following amendment has two parts. The first part (below) answers questions received. The second part (attached) is the amended RFP. The amended RFP replaces the old, and the old RFP is hereby void.

Questions received and answered Please see below for questions and answers pertaining to 36C26218R0615.

Q1. Is there an incumbent contractor currently performing these services?

A1. The Blanket Purchase Agreement number is VA262-17-A-0057, and the contractor name is Advanced Med, LLC. Any further information is unauthorized beyond this. If you require additional information, you may submit a Freedom of Information Act (FOIA) request to SDCVAMCFOIA@va.gov.

Q2. Factor 3 Past Performance: Can recent VA past performance within the last 6 months be submitted as relevant as the solicitation states "current or past within the past 3 years" A2. Yes Q3. Are resumes required and/or will they be accepted?

A3. No, resumes are not required and will not be considered.

Q4. Is a self-certified Veteran Own Small Business eligible to submit a proposal for this RFP?

A4. No, this requirement is set-aside for a Service-Disabled Veteran-Owned Small Business (SDVOSB).

Q5. Please provide further clarification concerning page 6 of RFP item six (6) Limitations on Subcontracting.

A5. This will be deleted from the RFP, thank you.

Q6. What is the working status of GSA Employees and Contract Employees? Who has supervisory authority?

A6. The government does not supervise contracted personnel. The contractor is responsible for supervising contracted personnel.

Q7. The RFP response is to be submitted by email. Is there a File Size limitation in sending the response to the RFP?

A7. 10 Mb should be more than enough, but if you have more, send in clearly-labeled, multiple emails.

Q8. Section B-4, Schedule of Services and Price: The Presolicitation Notice, indicates that this will be an IDIQ contract. Is the interpretation of this is that the Government will select a small group of contractor who would have to bid again on Tasks Order put out by the VA for the contract period?

Amendment to RFP 36C26218R0615 A00002 Page 2 of 3

A8. The will be a single-award contract, not multiple-award. Thus, the task orders would to be awarded to the same contractor as the IDIQ.

Q9. Two question were similar here:

a. What is the Level of Sensitivity for each of the CLINs identified in the Proposal?

b. Page 29 Para 41.4 - What level of background Investigation should we budget for for each modality?

A9. The level of sensitivity is “Low” for each modality.

Q10. SCT is subcontracting some of the work to an elite radiology staffing company out of New York - will the government evaluate Past Performance proposed by our subcontractor, so long as our subcontractor performed the work as a prime contractor on the referenced contract?

A10. No, the government will review the past performance of only the vendor who submits and offer to this RFP.

Q11. Page 10 Para 1.1 - Can you provide an estimate of required day, evening and night shifts projected requirements?

A11. Page 25, 31 states, Actual workdays and tours will be scheduled according to VA staffing needs. Some weeks we could have a need for all day, or all weekend, or evening. It is difficult to predict a clear estimate.

Q12. The following questions all pertain to a decreased standard and are answered together.

a. Page 15 Para 4.3 - Request "Contractor will use best efforts" to meet the 48 hour requirement. The proposed candidates do not work for us and therefore we cannot require them to respond within 48 hrs. In addition, the last sentence states all candidates shall start work within48 hrs of clearance notification. Do you anticipate needing all five candidates for each modality to be on the schedule within 48 hours of clearance?

b. Page 15 Para 4.4 - Request "Contractor will use best efforts" to meet the 48 hour requirement.

c. Page 15 Para 4.5 - Request "Contractor will use best efforts" to meet the 2 hour requirement. The only way to ensure this turnaround is to pay for this service by having one of the five providers on backup and paying them to be on backup. If providing a backup is acceptable, then request "Backup hrs be added to the pricing schedule (CLIN0008, CLIN1008, CLIN2008, CLIN3008, CLIN4008).

d. Page 26-27 Para 34 - Request deletion. The Contractor’s personnel may be released from duty after a minimum of four (4) hours if their services are no longer required, however contract personnel will only be paid for actual hours work. Any tour of duty for Contractor personnel may be canceled no less than twenty-four (24) hours prior to start of assigned shift without penalty to the Government if staffing permits.

e. Page 27 Para 37 - Request "Contractor will use best efforts" be added A12. The requirements, as stated in the RFP, reflect the governments need to care for the veteran client. We have reviewed those standards and feel they are properly put forth in the RFP.

Q13. Page 16 Para 9 - Request Deletion The liaison may be a contract hospitalist physician performing under the resulting purchase agreement."

A13. The RFP will be amended to replace “hospitalist physician” with “technician.” Thank you.

Q14. Page 17 Para 12 - Request a reasonable maximum amount (e.g. 10%) about negotiated rate.

A14. No, the government intends the standard to be as written.

Q15. 9. Page 26 Para 33 - Will providers be scheduled on holidays or be scheduled to be on-call?

A15. Yes, refer to section 31 of the RFP.

Q16. Is there someone in the facility that is responsible for scheduling the shifts for these technicians? Or is this something that the customer manages?

A16. The supervisor of the modality and/or the Chief Technologist will identify need and prepare work schedule relative to the needs of the Service. The contract is to meet our required staffing needs. Work shifts are states in Section 31.1 of the RFP.

Q17. How many FTEs will be required for this opportunity?

A17. The requirement is set by hours, not FTEs. We require the hours are achieved, regardless of how many FTE’s are used.

Q18. Section 1.1 States: Mammography Technologist (.25 FTE for base year) and (1.0 FTE for option years) and B.4 SCHEDULE OF SERVICES AND PRICE for Option Year 1-4 list 520 hours which is still .25 FTE. Will that position remain .25 FTE or be 1.0 FTE beginning the first option year?

A18. We anticipate the base year to be .25 FTE and potential to 1.0 FTE for the option years. Section B.4 SCHEDULE OF SERVICE AND PRICE is hereby amended to reflect this.

Q19. Can the on-call CLIN hours be used for both call back and on-call needs?

A19. No, the number of hours are identified as were the number of on-call hours under the Schedule of Services and Price, Page 7, B.4. On-call hours are considered separate from the number of hours required for each modality.

Q20. Would the Department be willing to kindly modify the Past Performance section to include Subcontractor Past Performance?

A20. No, we are only considering the past-performance of the prime vendor.

Q21. is this going to be SCA?

A21. Yes, the Service Contract Act applies.

Q22. Am I correct in believing that you would like to have at least 25 candidates put forward in order to ensure there is always backup?

A22. Yes, the expectation is that the Vendor will provide us with contract staff to meet our requested need and in order to do so they will need to have a sufficient pool of candidates in each of the listed modalities to draw from.
