PROJECT PURPOSE AND INTENT

1. This project shall provide comprehensive Design Build services to replace the existing generator diesel fuel storage tank and associated system components at Veterans Affairs Pacific Islands Healthcare Systems (VAPIHCS) CFA Building 110. The existing 200kW Emergency Generator provides power to critical circuits in the event of a natural disaster or power outage. During tank annular testing, a leak into the secondary wall of the fuel storage tank was identified. The existing equipment is over 25 years old, replacement of equipment is required.

2. The project is located at Krukowski Rd Building, 110, Honolulu, HI 96819. The fuel storage tank is located on the exterior of the CFA Generator House (Building 110A).

3. The scope of this project covers the land on the South-East of campus. See the attached drawings.

4. The Design Build (DB) contractor shall furnish:

a. Design: all design (AE) services, materials, supplies, equipment, investigations and project supervision, and construction period services associated with this contract. The Government will consider an Interim CQC Plan for the first 15 days of operation, which must be accepted within 5 business days of NTP. Construction will be permitted to begin only after acceptance of the CQC Plan or acceptance of an Interim plan applicable to the particular feature of work to be started. Work outside of the accepted Interim CQC Plan will not be permitted until acceptance of a CQC Plan or another Interim CQC Plan containing the additional work scope is accepted.

i. Content of the CQC Plan: Include, as a minimum, the following to cover all construction operations, including work by subcontractors, designers of record consultants, architects/engineers (A/E), fabricators, suppliers, and purchasing agents:

b. QC Manager (QCM): Identify as QC Manager an individual within the onsite work organization that is responsible for overall management of quality control and has the authority to act in all CQC matters for the Contractor. The QC Manager is required to be a graduate engineer, graduate architect, or a graduate of construction management, with a minimum of 5 years of QC Manager experience on a project with similar scope to this Contract. This QC manager is to be on the site at all times during construction and is employed by the Prime Contractor. Identify in the plan an alternate to serve in the event of the QC Manager’s absence. The requirements for the alternate are the same as the CQC System Manager. Submit to the Government an appointment letter signed by an authorized official of the firm which describes the responsibilities and delegates sufficient authorities to adequately perform the functions of the QC Manager, including authority to stop work which is not in compliance with the Contract. Along with the letter, furnish the QC Manager’s resume, certifications/qualifications, previous similar projects worked on. Should the QC Manager fail to perform their duties satisfactory, the Government reserves the right to require the Contractor to remove and replace personnel as necessary, at no additional cost to the Government. A copy of the letter to the QC Manager signed by an authorized official of the firm which describes the responsibilities and delegates sufficient authorities to adequately perform the functions of the QC Manager, including authority to stop work which is not in compliance with the Contract shall be submitted to the COR. Letters of direction to all other various quality control representatives outlining duties, authorities, and responsibilities will be issued by the QC Manager. Furnish copies of these letters to the Contracting Officer or Authorized designee.

c. Due to the size of this project, the SSHO, Project Superintendent, and Quality Control Manager responsibilities may be performed by a single Prime Contractor employee.

d. NOTIFICATION OF NONCOMPLIANCE: The Contracting Officer or Authorized designee will notify the Contractor of any detected noncompliance with the foregoing requirements. The Contractor should take immediate corrective action after receipt of such notice. Such notice, when delivered to the Contractor at the work site, will be deemed sufficient for the purpose of notification. If the Contractor fails or refuses to comply promptly, the Contracting Officer can issue an order stopping all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to such stop orders will be made the subject of claim for extension of time or for excess costs or damages by the Contractor.

a. The Design Build Contractor shall meet with VAMC’s COR to discuss and outline phasing requirements for the project. These phasing requirements shall describe the general sequence of the project work, estimated project duration, and what Government constraints will exist thatwill influence the Contractor's approach to the construction project. The Contractor shall be responsible for recording the phasing requirements. Parking impacts, pedestrian traffic, and OHV traffic should all be considered; the station cannot suffer construction impacts that unduly inhibit patient flow, or the site traffic flow.

b. Submit a phasing narrative and phasing plans (on reduced size plans) within two weeks after each phasing meeting to the VAMC COR. VA will review these submission(s) and return comments to the contractor within two weeks of receipt. The contractor will then use this information in preparing their schematic, design development, and construction document submissions.
