About this file
This is a Standard Form 30 Amendment of Solicitation/Modification of Contract issued by the Department of Veterans Affairs, Network Contracting Office (NCO) 21 for solicitation number 36C26126Q0684, originally dated May 1, 2026. The amendment extends the deadline for receipt of offers from the original date to May 21, 2026 at 10:00 AM PDT to allow time for responses and questions and answers. The contracting officer is Cynthia Diezel.
The amendment includes clarifications to the Performance Work Statement (PWS) through two Q&A responses. The first addresses the timeframe for autopsy case closures by updating Section 2.2.5 to specify that physicians must provide preliminary anatomical diagnosis within three (3) business days for all autopsies, and a final autopsy diagnosis report within sixty (60) business days, with copies including the Final Anatomical Diagnosis (FAD) to be faxed to (650) 496-2574 to the VAPAHCS Laboratory Director and originals mailed to the Chief of Pathology & Laboratory Medicine Service (P&LMS) at VA Palo Alto (3801 Miranda Avenue, Room E4-332, California 94304) via trackable mail. The second clarification addresses transportation logistics for deceased patients, confirming that the contractor's round-trip transport service from Bay Area mortuaries is to VA Palo Alto, with VA (not the contractor) responsible for releasing bodies to funeral homes, meaning pickup and return of decedents will be to VA Palo Alto and not directly to funeral homes.

CONTINUATION PAGE
The purpose of this amendment is to extend the solicitation date to May 21, 2026 at 10AM to provide responses to the solicitation and the provide questions and answers. The following questions are posted:
1. Q: May you please confirm the timeframe this section 5.1.2 is referencing is the timeframe in section 2.2.5?
We did not see a section 2.1.1.3 outlining any timeframe but did see timeframes listed in
Section 2.2.5.
A: Paragraph 2.2.5 will be updated to reflect the following:
2.2.5 Physician(s) shall provide the VAMC with preliminary anatomical diagnosis within three (3) business days for all autopsies. A final autopsy diagnosis report for all autopsies shall be provided within sixty (60) business days. Copies of these reports which shall include the Final Anatomical Diagnosis (FAD) shall be faxed to
(650) 496-2574 addressed to the VAPAHCS Laboratory Director as soon as they are available. Originals shall be mailed to the VAMC at: Chief, Pathology &
Laboratory Medicine Service (P&LMS) (113), 3801 Miranda Avenue, Room E4-332, California, 94304. Any mailed documents shall be trackable by a tracking number system.
- Above changes are in red text from the original PWS.
2. Q: B.2 Price/Cost Schedule lists the Transport Bay Area Mortuary services as
Round-Trip From and Back to VA Palo Alto, however section 6.1.3.2 states VAMC will provide Contractor with name and address that will receive the deceased. May you please clarify if the deceased patient would be transported back to VA Palo
Alto and VA Palo Alto will transport deceased patient to the identified funeral home, or would the Contractor be expected to directly transport patient to the identified funeral home upon completion of autopsy/release of body back to VA
Palo Alto?
A: While there will be some upcoming changes as Decedent Affairs transitions to be under Social Work and not Admitting and Eligibility, I believe the preference is for VA to release bodies to the mortuaries, so the pickup and return of the decedent will be to VA Palo Alto and *not* directly to a funeral home.
