About this file
This is an amendment (Amendment 0002) to solicitation 36C26125Q0227 for ground ambulance services at VA Southern Nevada Health Care System, containing questions and answers that clarify requirements. The amendment includes 24 questions and VA responses addressing operational details such as: expected daily patient volume (0-6 patients), no contractor compensation for maintaining four ambulances on standby, VetRide (not Tungsten) will be used for invoicing, and clarification that stretcher transports should not be billed at different rates from standard ALS/BLS/CCT transports.
Key clarifications include: this is a 100% small business set-aside (not SDVOSB/VOSB), contractors may post ambulances on VA property to meet 15-minute emergency response times, all transports require medical personnel, and contractors cannot downgrade transport levels requested by VA providers. The VA also clarified that out-of-state transports are rare (1-2 times per year), remote clinic transports from Laughlin/Pahrump typically use 911 services, and critical care monitors must have specific capabilities including continuous 12-lead EKG monitoring, 2-temperature monitoring, automatic blood pressure, capnography, and 2 arterial lines monitoring.

The purpose of this amendment is to include the Question and Answer (QA) response below.
All other terms and conditions remain unchanged.
Tim Smith Contracting Officer
1. What is the approximate amount of patients daily and their average mileage?
VA Response - Approximately 0-6 daily.
2. How many ambulances daily would be required?
VA Response - Approximately 0-6 daily.
3. Are the EMTs to stay and wait for the patient to do a return trip or is it strictly a drop off (outside of returning the VA health care employee).
VA Response - We do have some veterans that will need EMTs to stay and wait. These are usually scheduled in advanced.
4. Is this a new contract or was there a previous contract? If previous contract was held, whom and how much was the contract?
VA Response – There is an incumbent contractor in place. The remainder of the questions must be requested through the Freedom of Information Act.
5. According to the SF 1449, this solicitation is a set aside for a Total Small Business but, in the submission instructions on page 84 C. Volume 3, it states that this solicitation is a SDOSB/VOSB set aside. Would you please confirm which set aside actually applies?
VA Response – As listed in block 10 of the 1149, this requirement is a 100% small business set-aside.
6. Page 4 of the solicitation states that the FSC uses Tungsten for electronic invoice submission. Will the contractor be required to use Tungsten or VetRide for invoice submissions?
VA Response – VetRide will be used for invoicing. The contract resulting from this solicitation will be revised to remove verbiage relating to Tungsten.
7. Section 4.1.2 of the Performance Work Statement (PWS) requires the contractor to return VA staff to the original place of pick-up. Is the return trip billable under the contract? If so, should this be billed as a base rate or wait time charge?
VA Response – Contractor will not be reimbursed for the return trip if the VA staff and/or equipment is required to be used during transport. Items on the Price/Cost Schedule are the only services that can be invoiced and paid for.
8. The Price/Cost Schedule (Pages 6-8) lists emergency and non-emergency ALS transports but only non-emergency BLS transports. If a provider requests an emergency transport at the BLS level, is there a separate billing code for this, or are all BLS transports considered non-emergency? We will have only one price for BLS.
VA Response - We do not separate BLS.
9. The solicitation references "stretcher transports," (Sections 1.0, 4.1.1., 4.5.1.1., 4.5.1.4., 4.5.1.6., 4.5.2.1., etc.) but no line item exists on the Price/Cost Schedule (Pages 6-8) for these transports. Should these be billed at a different rate, or are they considered BLS transports?
VA Response - They shouldn't be billed at a different rate. The rates are only by the type of trip ALS, BLS, or CCT. Stretcher should need not be priced.
10. The solicitation references "stretcher transports." Can the contractor utilize non-medical personnel for these transports when the patient does not have a medical need?
VA Response - No, all transport through the ambulance company will have medical needs.
11. Section 4.5.1.6 (Page 22) requires the contractor to maintain sufficient vehicles and personnel to provide up to four (4) simultaneous transport requests. On days when there are fewer than four transports, will the government compensate the contractor for maintaining four ambulances on standby?
VA Response - No
12. Section 4.5.1.9.1 (Page 23) requires the contractor to be on scene at the scheduled transport time, but Section 6.2 (Page 29) allows for a 15-minute deviation. Please clarify the performance standard for scheduled transports.
VA Response - Schedule pick-up times must adhere to the times listed in the performance work statement (PWS). Page 29 contains standards found in the QASP.
The QASP states scheduled patient pick-ups shall not deviate more than 15 minutes past scheduled pick-up times for schedule orders. The acceptable quality level is 97% of the time. The QASP references Section 4.5.1.8.3 of the PWS but the correct reference is Section 4.5.1.9. “Response Times*”. This will be updated on the contract resulting from this solicitation.
13. Section 4.5.1.9 (Page 23) defines a scheduled transport as one with more than 4 hours’ notice and an unscheduled transport as one with less than 4 hours’ notice. If a transport is scheduled between 1-4 hours in advance (e.g., patient will be ready in 3 hours), is this classified as scheduled or unscheduled? Must the contractor be on scene at the scheduled time or within 1 hour of the scheduled time?
VA Response - Scheduled transport are transports scheduled more than 4 hours in advance for example: 4 hours and 01 minutes. Unscheduled transports are transports schedule 3 hours 59 minutes or less. Contractor is given an hour to arrive at the schedule time.
14. Section 4.5.2.2 (Page 24) states that the contractor must immediately dispatch an ambulance for out-of-network transports. Does this include transports from the VA campus to non-VA hospitals in Las Vegas? VA Response - Yes, we transfer veterans to no VA facility frequently. Does this also apply to long-distance transports (e.g., Las Vegas to Palo Alto)? VA Response - Yes. If so, can long-distance transports be scheduled in advance to accommodate crew rest periods and vehicle safety checks? VA Response - These transports are scheduled in advanced. It is very rare we send a veteran out of state by ambulance. It may happen 1-2 times a year.
15. Section 5.2.2 (Page 27) lists Community-Based Outpatient Clinics (CBOCs), two of which are 1 hour 45 minutes from the main VA campus (Pahrump VA Clinic & Master Chief Jesse Dean VA Clinic). Will additional response time be allowed for transports to/from these locations? VA Response - The staff at Laughlin/Pahrump typically contact
911. For Laughlin and Pahrump, we request ambulance transport for sending veterans home that medically require ambulance transport. We may have a veteran that need transporting to an appointment via ambulance in Laughlin/Pahrump. This is very rare but could happen 1-2 times a year. These transports will be scheduled in advance. Will the contractor be required to position ambulances in the vicinity of these clinics. If required, what is the expected call volume from these clinics? VA Response - It is not required to be in the vicinity. Will the government compensate the contractor on days when no transports occur from these clinics? VA Response - No.
16. Section 4.5.1.9.3 (Page 23) lists Urgent Care under response time requirements. Please define "Urgent Care" in this context, as it is typically associated with a walk-in clinic rather than an emergency transport requirement. Urgent Care is to describe what type of care is needed for the patient, walk in clinics can provide urgent care.
VA Response - It is care that is not emergent but does need medical assistance that cannot wait a certain amount of time. The provider decides the type of transport to request.
17. Section 4.5.1.9.5 (Page 23) requires the contractor to respond within 15 minutes for emergent or urgent-level calls. Will the VA allow contractor ambulances to post on the VA main campus to meet this requirement?
VA Response - Yes
18. If contractor ambulances are allowed to post on VA property for urgent response: Can they be integrated into the VA’s emergency response plan? VA Response - No. If a veteran collapses in the VA parking lot, can the contractor respond and transport the veteran to the VA ER?
VA Response - Emergency response should follow what they have been trained and licensed to do.
19. Section 4.5.1.9 (Page 23) differentiates between "Urgent Care" and "Urgent Care” deemed critical by provider." What is the difference between these two classifications?
What criteria does the VA provider use to determine critical status?
VA – The contractor will be notified if the request is for urgent care or urgent care deemed critical by the provider and will adhere to applicable response time listed in the performance work statement.
20. What is the difference between an "Urgent Transport" and an "Emergent Transport" as described in Section 4.5.1.9 (Page 23)?
VA Response - Urgent transport is not in the PWS, urgent care is in the PWS. Emergent Transport is classified as critical care/code 3.
21. Attachment A specifies the Zoll X Series Critical Care Monitor. Can the contractor use other critical care monitors such as Philips Tempus, LifePak 35/15, or Philips MRx?
VA-Response - The equipment must have these features: Continuous 12-lead EKG monitoring, 2-Temperature Monitoring, Automatic Blood Pressure, Capnography, 2 arterial lines monitoring.
22. Attachment A Drug Supply List. Are contractor’s critical care personnel required to carry these drugs, or can they be provided by the sending facility if needed during transport?
VA Response - The contractor shall not borrow or exchange supplies, equipment, and/or medications with the VA. Equipment attached to or in use by the patient at time of pick-up may remain with the patient during transport to ensure continuity of care. Medical Control Authorities are responsible for maintaining and supplying medication during patient transport. All VA equipment and unused medications must be returned to VA during patient drop-off or as promptly as feasible within 24 hours.
23. Page 37 requires a "Licensed MTO" to oversee vehicle sanitation. Please define "Licensed MTO" and clarify if this refers to a Medical Transport Officer or another role.
VA-Response - The EMT that is selected to oversee vehicle sanitation.
24. Who determines the level of care required for ambulance transport? VA Response - The provider. What reference material or medical guidelines does the VA follow? VA Response - The VA follow medical protocols. If a VA provider requests a higher level of care than appears necessary, can the contractor downgrade the transport to an appropriate level?
VA Response - No
