The purpose of this amendment is to provide answers to questions received. Please see page 2 of this amendment for answers to questions received.

Questions and Answers

1. Does the VA want installation included in the quote?

Answer: Installation is not needed, these are on wheels and we can roll them right into the department.

2. Proposal Submission Format:

Could you please confirm the preferred format for submitting the proposal? Is there a specific template or structure you would like us to follow?

Answer: Quotation can be submitted as a quote on company letterhead or by filling out request for quote pages 1-7, no specific template or structure is preferred.

3. Electronic Submission Details:

Are there any specific requirements or restrictions for the electronic submission of the proposal, such as file size limits or acceptable file formats (e.g., PDF, Word)?

Answer: Accepted formats include Portable Document Format (PDF), MS Word, and Microsoft Excel. Note that there is a 20 MB file size limit per e-mail message.

4. Amendment Acknowledgment:

How should we acknowledge any amendments to the solicitation in our proposal? Is there a specific section where this should be noted?

Answer: Amendments can be acknowledged by email when sending the quote or if quote is provided by filling in the request for quote, amendments can be acknowledged on page 4.

5. Proposal Contents:

Besides the technical description, price, and past performance information, are there any additional documents or certifications required to be included with the proposal?

Answer: Please provide technical information, delivery information, pricing, business size and status, SAM Unique Entity ID, company address, and company POC information.

6. Equivalent Items:

If we propose an equivalent product to the specified TBJ models, what documentation or evidence do you require to demonstrate that it meets the salient characteristics?

Answer: We will require the technical data with spec sheet as well as drawings with proper measurements and requirements for Engineering to review and confirm that our facility electrical grid can support without having to complete a construction project.

7. Product Samples:

Are product samples required for this solicitation? If so, when and how should these samples be submitted?

Answer: No.

8. Delivery Timeline:

Could you clarify the expected delivery timeline for each item? Should we propose a timeline, or is there a specific timeframe that must be adhered to?

Answer: Please propose a timeline.

9. Shipping Instructions:

Are there any special shipping instructions, such as preferred carriers or specific packaging requirements, to ensure safe delivery to the VA Medical Center?

Answer: The facility has a loading dock, assuming due to the size and weight, these would go freight ground. We ask that they are palletized & wrapped to prevent any damage during shipping. Normally there is minor assembly required to install the pegboards on units that have the backing. Since they are separate pieces, this backing is either crated or shipped on a separate pallet.

Dock Info:

Hours of operation: Monday thru Friday 0800-1500 PST Closed Holidays

Any delivery exceptions (size of truck)? None, facility has dock
