CONTINUATION PAGES
1. Is the due date for the submission of the Quote to you via email on August 29, 2024 at 12:00 PDT? Please confirm.
Yes, that is the correct due date.
2. On page 8 Section 3.1 of the Solicitation is says:
3.1. SCHEDULE: The Contractor shall provide services 24 hours a day, 7 days per week, including federal holidays, as required by VA. All services shall be provided within the timeframes specified in Section 4 below. The Contractor shall provide routine scheduled specimen pickup at a time mutually agreed upon by the COR and the Contractor.
The VEXAS test is a low-volume, non-urgent, test and the number of people to be tested for the VEXAS mutation per year is about 24 subjects. The test would not require 24 hours a day / 7 days a week but would be done during regular work hours during the week and not on holidays.
Is the schedule considered applicable or not applicable?
The schedule will be modified to read:
3.1 SCHEDULE: The Contractor shall provide services during normal business hours of the normal work week. Services during weekends and federal holidays is not required. All services shall be provided within the timeframes specified in Section 4 below. The Contractor shall provide routine scheduled specimen pickup at a time mutually agreed upon by the COR and the Contractor.
3. In E.1 Addendum to FAR 52212-1 instructions to Offerors-Commercial items:
a) Is there a Form Template that we are supposed to use to prepare the submission of the Quote and provide the answers to the 4 Factors? We need to submit Factor 1:
Technical Capability; Factor 2: Past Performance; Factor 3: Socio-Economic Status;
and Factor 4: Price
There is no specific form or template. You may submit in any preferred format, but please make sure all factors are addressed.
b) For Factor 2: Past Performance: Do I provide references of past performance for our
REDACTED test or other tests that we offer in our Clinical Reference Laboratory or do you need a reference for the VEXAS test only?
You may provide references for any type of reference lab services.
c) For Factor 4: Price: It states in the Solicitation that Standard Form 30 (if applicable) shall be submitted with your solicitation. Where do I find Standard Form 30 to submit with my quote, if necessary?
Standard Form 30 (SF30) is a form that is used for solicitation amendments.
Amendments to the solicitation would need to be reviewed and signed.
Interested vendors are notified when an amendment is posted.
We also are supposed to do the FAR 52.212-3 Offerors Representations and Certifications (Reps & Certs), FAR 52.209-5 Certification Regarding Responsibility Matters, and FAR 52.209-7 Information Regarding Responsibility Matters (if applicable): If we are registered and up to date in the SAM System, are these already done?
They are generally completed in SAM.gov, but it is advisable to ensure the system does have the information and NAICS codes, small business information, etc. are up to date.
I believe the FAR 52.212-3 Offerors Representations and Certifications (Reps & Certs) is completed in the SAM System.
Where do we do the FAR 52.209-5 Certification Regarding Responsibility Matters, and FAR 52.209-7 Information Regarding Responsibility Matters? Are these already done in the SAM System?
These are also completed within SAM.gov and should already be completed if the information is up to date. It would be worthwhile to ensure the information is within SAM.gov.
4. For E.3 52.204-24 REPRESENTATION REGARDING CERTAIN
TELECOMMUNICATIONS AND VIDEO SURVEILLANCE SERVICES OR EQUIPMENT
(NOV 2021):
Is this certification already completed in the SAM System? If not, how do I represent that REDACTED ‘‘does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument’’ in paragraph (c)(1) in the provision at 52.204–26, Covered Telecommunications Equipment or Services—Representation, or in paragraph (v)(2)(i) of the provision at 52.212–3, Offeror Representations and Certifications–Commercial Products and Commercial Services.
This one is also within SAM.gov under 52.212-3 paragraph (v)(2)(i).
5. I am not sure if I have to complete the following representations mentioned on page 55 of the solicitation:
(c) Offerors must complete the following representations when the resulting contract is for supplies to be delivered or services to be performed in the United States or its outlying areas, or when the contracting officer has applied part 19 in accordance with 19.000(b)(1)(ii).
Has the Contracting Officer applied part 19 in accordance with 19.000(b)(1)(ii)?
This is also completed within SAM.gov under 52.212-3 paragraph (c). FAR 19.000(b)(1)(ii) is applied outside the United States and is not applicable to this requirement.
6. Do we sign the STANDARD FORM 30 in addition to the signed original STANDARD
FORM 1449, when we submit the Quote? Or just the STANDARD FORM 30? Please confirm.
You would sign both Standard Form (SF) 1449 and SF 30. Any other released SF 30s would require signature as well.
7. On page 24 B.3 PRICE/COST SCHEDULE: 3.1. SCHEDULE:
Do we cut and paste the ITEM INFORMATION Table provided on page 24 into our Quote that we will submit, with the unit price and amount included in the Table? Do we use this Table to answer Factor 4 – Price section?
Correct, you should insert your pricing into the Item Information Table provided on Page 24-25. That will cover the Factor 4 submission requirements.
8. In E.1 Addendum to FAR 52212-1 instructions to Offerors-Commercial items:
For Factor 3: Socio-Economic Status: We are not a SDVOSB or VOSB. We will not be subcontracting any work. Do I just briefly state this?
Correct, a brief statement will satisfy the requirement.
