DEPARTMENT OF VETERANS AFFAIRS

Justification for Brand Name IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C261-24-AP-3280

1. Contracting Activity: Department of Veterans Affairs (VA), VISN 21 , VA Northern California Health Care System, located in Mather, California. The purchase request, PR number, is 612-24-3-145-0165 ,

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

New requirement. The action being reviewed is the proposed purchase of the NIM 4.0 nerve monitoring system. The Medtronic NIM nerve monitoring system is used during ENT surgeries, including Thyroid, Parathyroid, Parotid and Ear cases. The unit itself is plug and play requiring minimal set up time which will continue to allow the operating room to meet division metrics. The tones on the unit can be easily programmed to identify differences in intraoperative nerve monitoring to identify varying nerves within operative windows in otologic, thyroid and facial surgery allowing for audible and visual cues to protect patients’ nerves. The system provides enhanced artifact noise suppression to reduce the number of false alarms or interference that can mask/hide the audible alarms of working around nerves. The wireless design that reduces the impact of wires on the surgical field is relatively compact which will allow it to fit into the overcrowded operating room. It allows for remote control use of critical settings directly from the surgical field allowing for more control from the provider.

The items needed are 1. a console, 2. a power cord, 3. a patient interfacer, 4. a connector cable, 5. an interface adaptor, 6. cart, and 7. probes.

3. Unique characteristics that limit availability to only one Brand, with the reason no other Brand can be used:

The station partner has used Medtronic equipment in the past, they have used the NIM 3.0 for the past several years, and this NIM 4.0 purchase would replace it. The station partner has trialed other versions of similar nerve monitoring systems from different manufacturers in 2021 and 2022 and determined that those units did not meet the minimum needs of the requirement. Medical personnel are trained on the NIM equipment, and the equipment has proven to provide the necessary level of care for patients.
