STATEMENT OF WORK

Rotary Microtome

1. Scope of Work: Histology laboratory needs rotary microtomes and blades for patient specimen processing. They will use them to section off samples of tissue for pathology review.

2. Location of the Work: Pathology & Laboratory Medicine Service Deliver to:

3801 Miranda Ave.

Bldg. 100 E4-319 Histology Palo Alto, CA 94304

3. Deliverables:

Four (4) Leica RM2245 Semi-Automatic Microtomes.

Instrument will be delivered and installed by vendor at the location of work.

Following items will be delivered monthly, 12 deliveries annually:

4. Microtome instrument requirements

4.1. Instrument for clinical histology and histopathology applications.

4.2. Must be motorized or automatic with an option for manual operation. The purpose of acquiring this equipment is to reduce injuries of employees stemming from repetitive arm, shoulder, and wrist motions.

4.3. Quick clamping system for specimen holders.

4.4. Handwheel locking systems.

4.5. Digital panel able to calibrated controls make it easy to adjust a specimen to an exact zero position.

4.6. Programmable specimen retraction.

4.7. Able to alternate between trimming and sectioning function.

5. Maintenance, Training and Service

5.1. This contract should be structured as a cost for supply contract. Maintenance and repairs are included in pricing. A maintenance schedule will be set and followed by the manufacturer.

5.2. Operators’ training will be provided and all expenses for training will be paid for by vendor as part of purchase contract.

6. Period of Performance:

6.1. Base year plus four option years. Five-year total contract.

7. Performance Requirements:

7.1. Supplies must be delivered on a monthly schedule. No more than 2 deliveries delayed per fiscal year.

7.2. Once a need for repair is identified, it is expected the service representative would arrive within 24 hours.

7.3. If equipment is inoperable more than 3 working days replacement will be an option. This is to avoid extended periods of equipment down time.

7.4. Not meeting standards stated here can result in a poor performance rating in the VA Contract Performance Assessment Reporting System (CPARS). This can adversely affect future government contracts.
