Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($150K)

1. Contracting Activity:

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Radiographic and Fluoroscopic equipment inspection and testing.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

Based on the specialized information required for the Radiographic and Fluoroscopic Inspections and Testing of the SPECT/CT and PET/CT equipment, John T. Stalp LLC is the proposed Sole Source Vendor. John T. Stalp is uniquely qualified based on his previous work with the specific equipment located in the VISN, and his specialized educational background and certifications.

This purchase is in accordance with FAR13.106-1(b).

4. Description of market research conducted and results or statement why it was not conducted:

A search of VETBIZ under NAICS 541380 and keywords radiographic and fluoroscopic resulted in (0) vendors being identified which could provide the related services. Additionally, a sources sought notice was issued on 11/15/2017 and closed on 11/20/2017 with (1) responses only one response from the incumbent vendor.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
