About this file
This is a Statement of Work (SOW) for Blood Bank Reagents for the Boise VA Medical Center's Transfusion Medicine Department. The contractor must supply antigen and antibody-specific reagents compatible with the Ortho Vision Analyzer to safely match blood products for transfusion across multiple VA departments including Emergency, Surgery, Oncology, and Medical Units. The contract period is five years with a base year beginning April 1, 2026, and requires monthly deliveries (12 per year) of four specific reagent types: Ortho Daily QC (4 sets per delivery, 52 annually), 0.8% Resolve Panel A (1 set per delivery, 12 annually), Surgiscreen (4 sets per delivery, 52 annually), Affirmagen (4 sets per delivery, 52 annually), and Ortho 7% BSA (2 sets per delivery, 30 annually). All reagents must be maintained at 2-8°C storage temperature with typical expiration dates of 3-4 weeks upon arrival.
The contractor must track lot numbers and expiration dates, ensuring consecutive lot numbers arrive within two days of current lot expiration to prevent service disruptions. Quality control inspections are required upon receipt and before use, documenting color, clarity, and seal quality per AABB standards. The vendor must replace damaged reagents within 24 hours at no cost and replace any reagents failing quality control standards without additional charge. The work will be performed at the Boise VAMC PALMs laboratory in the Transfusion Medicine Department. No government-furnished equipment or information is required, and no security risks or non-disclosure requirements apply since patient information is not shared with the vendor.

STATEMENT OF WORK (SOW)
Blood Bank Reagents for Ortho Vision Analyzer
1. Contract Title. Transfusion Medicine Reagent for testing and quality control purposes.
2. Background.
Antigen and Antibody specific reagents are required to safely match blood products (from blood donations) to those that require such products for life sustaining measures. This effort is only provided in the Transfusion Services Department.
3. Scope.
Transfusion Medicine is required to support multidisciplinary departments from the Emergency Department, Surgery, Oncology and all the Medical Units. Without the services of a Transfusion Medicine Department each of these departments would not be operational at the level required. This work is across the VISN but is site specific, therefore each VAMC requires it own Transfusion Medicine Department.
4. Specific Tasks.
The contractor is required to track the lot number and expiration date of each specific reagent the Boise VAMC Blood Bank will receive. These reagents are made from cellular material and have a short window for expiration. The contractor is required to send the current lot number within days of the older lot number expiring. This ensures that the Blood Bank is never without vital reagents to match blood products.
5. Performance Monitoring Each reagent is inspected upon arrival and again when place into use and documented as part the American Association of Blood Banks (AABB) Quality Control plan. If a reagent arrives damaged the vendor will replace the product within 24 hours at no additional cost. Additionally, if the reagent does not meet Quality Control Standards it will be replaced by the vendor at no additional cost.
6. Security Requirements There are no security risks or requirements for this reagent. The reagent is a material used to determine the antigens found on human cells and antibodies found in human plasma. Patient information is not shared with the vendor.
7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI).

8. Other Pertinent Information or Special Considerations.

d. Packaging, Packing and Shipping Instructions.
Reagents are packaged and shipped to be received within the identified storage temperature of 2-8◦ C. They typically have an expiration date of 3-4 weeks upon arrival and the next consecutive lot number will arrive within two days of the current lot number expiration.
e. Inspection and Acceptance Criteria.
Color, clarity, and seal quality is inspected and documented upon receipt of each reagent. The Lot number is logged in and each reagent is tested for quality control measures.
9. Risk Control Transfusion Medicine Reagent is made from human cells and is testing for infectious agents before being placed into production. However, universal precautions are used with all blood products including reagent.
10. Place of Performance.
Transfusion Medicine Reagent will be used in the Boise VAMC, PALMs laboratory, Transfusion Medicine Department.
11. Period of Performance.
Five-year contract with base year beginning 04/01/2026.
12. Delivery Schedule.
Schedule is dependent on lot specific information. There are 12 deliveries per year, one per month.
Ortho Daily QC – 4 reagent sets per delivery (52/YR) 0.8% Resolve Panel A – 1 reagent set per delivery (12/YR) 0.8% Surgiscreen – 4 reagent sets per deliveries (52/YR) 0.8% Affirmagen – 4 reagent sets per delivery (52/YR) Ortho 7% BSA – 2 every delivery (30/yr)
