B.2 PRICE/COST SCHEDULE
I. All hourly prices reflect the service rate the prime contractor will bill the VA for service work. Service rates will include the service truck/tools required for the specific trade journeyman inclusive of the hourly billed rate. All hourly rates are to include overhead, insurance, profit, and any other mark-up.
II. Each Task order will allow for one hour of billable travel time for each craft worker dispatched to the Puget Sound VA Medical Center designated site at the appropriate rate.
III. The Straight Time Rate will be billed for hours worked from Monday through Friday 7:00 a.m. to 4:30 p.m.
The Overtime Rate will be billed for hours worked from Monday through Friday 4:30 p.m. to 7:00 a.m. or, for anytime worked on Saturday.
The Sunday Rate will be billed for any time worked on Sunday.
The Holiday Rate will be billed for any time worked on any federal holiday.
Example of when to bill for travel time: If two journeyman Electronics Technicians are dispatched to work at the Puget Sound VA Medical Center on Sunday, then, one hour of travel time would be billed at the, “Electronics Technician Sunday Rate” for each of the two electricians. If two electricians are dispatched to work at the Puget Sound VA Medical Center during straight time, then, one hour of travel time would be billed at the, “Electronics Technician Straight Time Rate” for each of the two Electronic Technicians.

B.3 STATEMENT OF WORK
1. Contract Title: Network Cabling Services
2. General Information: This is a non-personal services contract to provide Network Cabling Services for the VA Puget Sound Health Care System. The Government shall not exercise any supervision or control over the contract service providers performing the service herein. Such contract service providers shall be accountable solely to the Contractor, who in turn is responsible to the Government.
3. Description of Services/Scope: The Contractor shall furnish all necessary parts, materials, labor, tools and equipment, and travel as necessary to provide Network Cabling Services 24 hours per day, 7 days per week, 365 days per year. Services rendered under this contract shall include only labor associated to support daily telecommunications operations, work order demands, unplanned office moves/projects.

All Existing and Future Satellite sites connected with VA Puget Sound Health Care System
· Bellingham, Mt. Vernon, Everett, Silverdale, Chehalis, Dupont, Renton, Seattle, Federal Way, Port Angeles, Magnolia, Puyallup, Edmonds, Lacey, Olympia, etc.
5. Period of Performance: See Section B.2 Price/Cost Schedule.
6. Specific Tasks: Cabling tasks may include but not limited to the following
6.1 Standard copper horizontal cable installations include CAT6A 4 pair cabling.
6.2 Pull and terminate CAT6A data cabling to 568B Standard. These cables will be pulled, terminated, and tested.
6.3 Install, move, add, change, and troubleshoot telephones and repair of inside wiring and telephones are required. The wiring consists of riser cables, Category 6E and 6 UTP, single and multi-mode fiber optic cable, racks, patch panels, and grounds
6.4 Pull and terminate fiber optic cabling with LC fiber connectors. These cables will be pulled, terminated, and tested.
6.5 Work on Audio visual equipment not limited to Smart Boards, Projectors, TV’s, and Patient TV’s. Cable Television (CATV) and Close Circuit Television (CCTV) cabling.
6.6 Work on Fax Lines including, but not limited to, troubleshooting, VOIP and analog cross connections.
6.7 Heating, Ventilation and Air Conditioning (HVAC) requirements Install, troubleshoot and repair Refrigeration alarm system.
6.8 Grounding and bonding.
6.9 Satellite Cabling. These cables will be pulled from site designated rack location in communications server room via conduit to closet in annex. Perform all cable terminations and testing.
6.10 Be able to work at various locations doing voice, data and electrical Including remote sites (away from 2 main campuses) and additional bus sites.
6.11 Purchase, assembly, install and labeling of cables, fibers, cable tray, cable management, cable support devices and systems.
6.12 Purchase, assembly and install of racks, cabinets, power distribution units, and for any new hardware.
7. Personnel Requirements:
7.1 The Contractor has the ultimate responsibility for managing the order, for achieving the expected performance results, and for determining the appropriate staffing in support of its technical approach.
7.2 The Contractor shall provide experienced and certified personnel to perform the required services.
7.3 Contractor personnel shall conform to standards of conduct and code of ethics, which are consistent with those applicable to Government employees. Contractor personnel shall obtain authorization to have access to Agency support sites and Government facilities.
7.4 All Contractor personnel shall have required tools and test equipment. The Government will not provide tools and test equipment.
7.5 All Contractor employees must be fluent in spoken and written English.
7.6 All Contractor personnel shall be United States Citizens.
7.7 The Contracting Officer may require the offeror to remove from the job site any Contractor employee who is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. In cases other than one of immediate need a written request stating the reasons for the removal should be provided 24 hours in advance to the offeror by the proper authorities.
7.8 The Contractor supplied personnel are employees of the Contractor and under the administrative control and supervision of the Contractor. The Contractor, through its personnel, shall perform the tasks prescribed herein. The offeror must select, supervise, and exercise control and direction over its employees (including subcontractors) under this order. The Government shall not exercise any supervision or control over the offeror in its performance of contractual services under this order. The offeror is accountable to the Government for the action of its personnel.
7.9 Changes or modifications to the SOW can only be made by the Government Contracting Officer.
7.10 The Contractor shall be qualified professionals with strong technical cable and wiring backgrounds and experience in performing large, complex cable and wiring installations.
7.11 The Contractor shall ensure that its employees have all required professional certifications and licenses (current and valid) for each applicable task and labor type category before commencement of work.
7.12 The Contractor shall identify, by name, the proposed Key Personnel (e.g., the key management and technical personnel who will work under this order, such as the PM).
7.13 The Contractor must meet facilities access control list (ACL) requirements to ensure access into server rooms and communication closet and has a security attribute that identifies its access control list. The list has an entry for each system user with access privileges. The Contractor must furnish a Licensed Journeyman Electrician with this contract. The Contractor must supply a confident Fiber worker as determined by the COR. The offeror must supply a confident Cisco switch programmer as determined by the COR.
8. Hours of Operation: Network Cabling shall be done during regular working hours of 9:00 am through 5:00pm, Monday – Friday, except weekends and Federal Holidays. Occasionally the contractor will be required to work weekends and extended hours to support scheduled and unscheduled projects such as cutovers, circuit maintenance, equipment upgrades, and emergency cable and equipment repairs. The contractor will also need to be on-call to respond to emergencies after normal business hours within four (4) hours or less of a call by VAMC Puget Sound for unscheduled or emergency onsite service.
Unscheduled service calls shall be invoiced on hourly basses.
The Contractor acknowledges that failure to respond to an emergency call could affect patient care and hospital operations. The Contractor agrees to make his best effort to respond to an emergency calls for Generator service as quickly as possible.
The eleven holidays observed by the Federal Government are as follows:

9. Security Requirements: The Contractor employees shall not have access to Veteran Affairs sensitive or computer information and will not require routine access to Veteran Affairs Facilities. The Contractor employees shall require intermittent access only and will be escorted by Veteran Affairs employees while at Veteran Affairs Facilities. No background investigation is required. The contract employee will require unescorted access to facility and key access throughout the facility including highly secured areas such as server rooms. All documentation (i.e., DD 254) required for security certification will be the responsibility of the offeror and the client organization.
10. Invoicing: VA published the final rule requiring vendors to submit invoices electronically to the Financial Services Center (FSC) in the November 27, 2012 Federal Register. The rule became effective December 27, 2012. The rule includes a new contract clause to be inserted in all solicitations and contracts by the contracting officer (VAAR 852.232-72). Vendors can comply with the rule by using either of the two methods below:
The FSC uses a third-party Contractor, Tungsten Network, to transition vendors from paper to electronic invoice submission. For information on Tungsten Network electronic invoicing set-up, vendors should call 1-877-489-6135, or email VA.Registration@Tungsten-Network.com.
Vendors are required to register with Tungsten Network and submit invoices electronically as a condition of acceptance of this contract or order.

C.2 52.216-18 ORDERING (AUG 2020)
(a) Any supplies and services to be furnished under this contract shall be ordered by issuance of delivery orders or task orders by the individuals or activities designated in the Schedule. Such orders may be issued from 1 January 2022 through 31 December 2026.
(b) All delivery orders or task orders are subject to the terms and conditions of this contract. In the event of conflict between a delivery order or task order and this contract, the contract shall control.
(c) A delivery order or task order is considered "issued" when—
(1) If sent by mail (includes transmittal by U.S. mail or private delivery service), the Government deposits the order in the mail;
(2) If sent by fax, the Government transmits the order to the Contractor’s fax number; or
(3) If sent electronically, the Government either—
(i) Posts a copy of the delivery order or task order to a Government document access system, and notice is sent to the Contractor; or
(ii) Distributes the delivery order or task order via email to the Contractor’s email address.
(d) Orders may be issued by methods other than those enumerated in this clause only if authorized in the contract.

*This is a 60-month (5 year) IDIQ from 1 January 2022 – 31 December 2026. The Government guarantees a minimum of $500.00 and this IDIQ is capped at a maximum of $1,500,000.00 over the 5 year ordering period.*
(a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than $500.00, the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract.
(b) Maximum order. The Contractor is not obligated to honor—
(1) Any order for a single item in excess of $250,000.00;
(2) Any order for a combination of items in excess of $1,500,000.00; or
(3) A series of orders from the same ordering office within 1 days that together call for quantities exceeding the limitation in paragraph (b)(1) or (2) of this section.
(c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section.
(d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 2 days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source.

C.4 52.216-22 INDEFINITE QUANTITY (OCT 1995)
(a) This is an indefinite-quantity contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies and services specified in the Schedule are estimates only and are not purchased by this contract.
(b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. The Contractor shall furnish to the Government, when and if ordered, the supplies or services specified in the Schedule up to and including the quantity designated in the Schedule as the "maximum." The Government shall order at least the quantity of supplies or services designated in the Schedule as the "minimum."
(c) Except for any limitations on quantities in the Order Limitations clause or in the Schedule, there is no limit on the number of orders that may be issued. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations.
(d) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after 31 December 2026.

C.5 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days of the end of the contract.

C.6 SUPPLEMENTAL INSURANCE REQUIREMENTS
In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:
(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.
(b) General Liability: $500,000.00 per occurrences.
(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.
(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.

(a) This solicitation includes VAAR 852.219-11 VA Notice of Total Veteran-Owned Small Business Set-Aside.
(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement.

(d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
