B.3 STATEMENT OF WORK (SOW)

1. Contract Title: Vancouver Campus Boiler Plant Source Testing, 1601 E. Fourth Plain Blvd., Vancouver, WA 98661.

2. Description of Services/Scope: The contractor shall provide all necessary equipment, labor, materials and supervision to complete all work required to provide boiler plant source testing as stated in the SOW and SOW Attachment SWCAA Air Discharge Permit 4-2019.

3. Place of Performance: Veteran Affairs Portland Health Care System – Vancouver Campus (648), 1601 E. Fourth Plain Blvd., Vancouver, WA 98661.

4. Period of Performance: 60 days from date of award

5. Specific Tasks: The contractor shall provide materials, equipment, labor and parts to provide Vancouver Boiler Plant Source Testing at the VA Portland Health Care System, Vancouver Campus.

a. Per Washington State regional Southwest Clean Air Agency ( SWCAA ) air discharge quality permit, ( attached ) the following 40 CFR 60 testing is required per Appendix A, and B.

i. EPA Methods 1 and 2

ii. EPA method 3A

iii. EPA Method 4 – 60 minutes

iv. EPA Method 7E – 60 minutes

v. EPA Method 10 – 60 minutes

vi. SWCAA Method 9: Visible Emissions from boiler operation must not exceed zero percent opacity for more than the 3 minutes in any one-hour period as determined in accordance with SWCAA Method 9 ( See Appendix A of SWCAA 400 ).

b. A comprehensive test plan must be submitted to SWCAA for review and approval at least 10 calendar days PRIOR to testing.

c. SWCAA must be notified of the test date at least 5 calendar days prior to testing.

d. Unless otherwise specified, for each boiler, testing for each constituent must consist of a minimum of three sampling runs of the duration specified by Air Discharge Permit 19-3332 Appendix A, Page 1.

e. The results of all required testing must be submitted to SWCAA within 45 days of test completion. Unless otherwise directed by SWCAA, a single hard copy of the report, and an electronic copy of the report must be submitted. A hard copy and an electronic copy will be delivered to the VA as well. Please ensure all items from Air Discharge Permit 19-3332 Appendix A Item 4 are included in the report.

6. Security Requirements: The Contractor employees shall not have access to Veteran Affairs sensitive or computer information and will not require routine access to Veteran Affairs Facilities. The Contractor employees shall require intermittent access only and will be escorted by Veteran Affairs employees while at Veteran Affairs Facilities. No background investigation is required.

7. Invoicing: VA published the final rule requiring vendors to submit invoices electronically to the Financial Services Center (FSC) in the November 27, 2012 Federal Register. The rule became effective December 27, 2012. The rule includes a new contract clause to be inserted in all solicitations and contracts by the contracting officer (VAAR 852.232-72). Vendors can comply with the rule by using either of the two methods below:

a. 1. The FSC uses a third-party contractor, Tungsten Network, to transition vendors from paper to electronic invoice submission. For information on Tungsten Network electronic invoicing set-up, vendors should call 1-877-489-6135, or email VA.Registration@Tungsten-Network.com.

b. Vendors are required to register with Tungsten Network and submit invoices electronically as a condition of acceptance of this contract or order.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
