B.3 STATEMENT OF WORK
1. Contract Title: Network Cabling Services
2. General Information: This is a non-personal services contract to provide Network Cabling Services for the Mann-Grandstaff Veteran Affairs Medical Center. The Government shall not exercise any supervision or control over the contract service providers performing the service herein. Such contract service providers shall be accountable solely to the Contractor, who in turn is responsible to the Government.
3. Description of Services/Scope: The Contractor shall furnish all necessary parts, materials, labor, tools and equipment, and travel as necessary to provide Network Cabling Services 24 hours per day, 7 days per week, 365 days per year.

5. Period of Performance: See Section B.2 Price/Cost Schedule.
6. Specific Tasks: Cabling tasks may include but not limited to –
6.1 Standard copper horizontal cable installations include CAT6 4 pair cabling
6.2 Pull and terminate Cat #6 data cabling to 568B Standard. These cables will be pulled, terminated, and tested
6.3 Install, move, add, change, and troubleshoot telephones and repair of inside wiring and telephones are required. The wiring consists of riser cables, Category 6E and 6 UTP, single and multi-mode fiber optic cable, racks, patch panels, and grounds
6.4 Pull and terminate 50-micron fiber optic cabling with ST fiber connectors. These cables will be pulled, terminated, and tested
6.5 Work on Audio visual equipment not limited to Smart Boards, Projectors, TV’s, and Patient TV’s. Cable Television (CATV) and Close Circuit Television (CCTV) cabling
6.6 Work on Fax Lines including, but not limited to, troubleshooting, VOIP and analog cross connections using 66, 110, and, krone cross connectors
6.7 Troubleshoot and repair medication carts and all systems within the cart
6.8 Heating, Ventilation and Air Conditioning (HVAC) requirements Install, troubleshoot and repair Refrigeration alarm system
6.9 Grounding and bonding
6.10 Satellite Cabling. These cables will be pulled from site designated rack location in communications server room via conduit to closet in annex. Perform all cable terminations and testing
6.11 Be able to work various locations doing data and electrical Including Wenatchee, CDA, Downtown Homeless, outreach, Plummer Idaho and additional bus sites
6.12 Troubleshoot and Install PA systems as well as any ancillary parts/fixtures conducive to proper operation of the equipment
6.13 Cable tray cable support devices and systems
6.14 Racks and cabinets for new hardware
6.15 Power requirements – Dedicated circuits Installation of Electrical Outlets to current medically approved standards
7. Personnel Requirements:
7.1 The Contractor has the ultimate responsibility for managing the order, for achieving the expected performance results, and for determining the appropriate staffing in support of its technical approach.
7.2 The Contractor shall provide experienced and certified personnel to perform the required services.
7.3 Contractor personnel shall conform to standards of conduct and code of ethics, which are consistent with those applicable to Government employees. Contractor personnel shall obtain authorization to have access to Agency support sites and Government facilities.
7.4 All Contractor personnel shall have required tools and test equipment. The Government will not provide tools and test equipment.
7.5 All Contractor employees must be fluent in spoken and written English.
7.6 All Contractor personnel shall be United States Citizens.
7.7 Background Checks: All employees must submit a Questionnaire for National Security Positions (SF-86) to the [Agency] Personnel Security Manager. A favorable SF-86 is required before gaining access to a U.S. Government Local Area Network (LAN), if required. The offeror, when notified of an unfavorable determination by the Government, shall withdraw the employee from consideration from working under the order.
7.8 The Contracting Officer may require the offeror to remove from the job site any Contractor employee who is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. In cases other than one of immediate need a written request stating the reasons for the removal should be provided 24 hours in advance to the offeror by the proper authorities.
7.9 In order to ensure a smooth and orderly startup of work, it is essential that the key personnel specified in the Contractors quote be available on the effective date of the order. If these personnel are not made available at that time agreed upon, the offeror must notify the Contracting Officer 5 days in advance to show cause. If the offeror does not show cause, the offeror may be subject to default action.
7.10 The Contractor supplied personnel are employees of the Contractor and under the administrative control and supervision of the Contractor. The Contractor, through its personnel, shall perform the tasks prescribed herein. The offeror must select, supervise, and exercise control and direction over its employees (including subcontractors) under this order. The Government shall not exercise any supervision or control over the offeror in its performance of contractual services under this order. The offeror is accountable to the Government for the action of its personnel.
7.11 Changes or modifications to the SOW can only be made by the Government Contracting Officer.
7.12 The Contractor shall be qualified professionals with strong technical cable and wiring backgrounds and experience in performing large, complex cable and wiring installations.
7.13 The Contractor shall ensure that its employees have all required professional certifications and licenses (current and valid) for each applicable task before commencement of work.
7.14 The Contractor shall identify, by name, the proposed Key Personnel (e.g., the key management and technical personnel who will work under this order, such as the PM).
7.15 The Contractor must meet facilities access control list (ACL) requirements to ensure access into server rooms and communication closet and has a security attribute that identifies its access control list. The list has an entry for each system user with access privileges. The Contractor must furnish a Licensed Journeyman Electrician with this contract. The Contractor must supply a confident Fiber worker as determined by the COR. The offeror must supply a confident Cisco switch programmer as determined by the COR.
8. Hours of Operation: Network Cabling shall be done during regular working hours of 9:00 am through 5:00pm, Monday – Friday, except weekends and Federal Holidays. Occasionally the contractor will be required to work weekends and extended hours to support scheduled and unscheduled projects such as cutovers, circuit maintenance, equipment upgrades, and emergency cable and equipment repairs. The contractor will also need to be on-call to respond to emergencies after normal business hours within four (4) hours or less of a call by VAMC Spokane for unscheduled or emergency onsite service.
The Contractor acknowledges that failure to respond to an emergency call could affect patient care and hospital operations. The Contractor agrees to make his best effort to respond to an emergency calls for service as quickly as possible.

9. Security Requirements: The Contractor employees shall not have access to Veteran Affairs sensitive or computer information and will not require routine access to Veteran Affairs Facilities. The Contractor employees shall require intermittent access only and will be escorted by Veteran Affairs employees while at Veteran Affairs Facilities. No background investigation is required. The contract employee will require unescorted access to facility and key access throughout the facility including highly secured areas such as server rooms. All documentation (i.e., DD 254) required for security certification will be the responsibility of the offeror and the client organization.
10. Invoicing: VA published the final rule requiring vendors to submit invoices electronically to the Financial Services Center (FSC) in the November 27, 2012 Federal Register. The rule became effective December 27, 2012. The rule includes a new contract clause to be inserted in all solicitations and contracts by the contracting officer (VAAR 852.232-72). Vendors can comply with the rule by using either of the two methods below:
The FSC uses a third-party Contractor, Tungsten Network, to transition vendors from paper to electronic invoice submission. For information on Tungsten Network electronic invoicing set-up, vendors should call 1-877-489-6135, or email VA.Registration@Tungsten-Network.com.
Vendors are required to register with Tungsten Network and submit invoices electronically as a condition of acceptance of this contract or order.

FSC and VA’s Office of Acquisition, Logistics and Construction (OALC) will assist existing commercial vendors in migrating to the electronic process. Until the transition to electronic format is complete, FSC will continue to process paper invoices for commercial vendors.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

C.4 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor at least 30 days prior to contract expiration.

C.5 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days of expiration of the contract; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.

C.6 52.223-5 POLLUTION PREVENTION AND RIGHT-TO-KNOW INFORMATION (MAY 2011)
(a) Definitions. As used in this clause— "Toxic chemical" means a chemical or chemical category listed in 40 CFR 372.65.
(b) Federal facilities are required to comply with the provisions of the Emergency Planning and Community Right-to-Know Act of 1986 (EPCRA) (42 U.S.C. 11001-11050), and the Pollution Prevention Act of 1990 (PPA) (42 U.S.C. 13101-13109).
(c) The Contractor shall provide all information needed by the Federal facility to comply with the following:
(1) The emergency planning reporting requirements of Section 302 of EPCRA.
(2) The emergency notice requirements of Section 304 of EPCRA.
(3) The list of Material Safety Data Sheets, required by Section 311 of EPCRA.
(4) The emergency and hazardous chemical inventory forms of Section 312 of EPCRA.
(5) The toxic chemical release inventory of Section 313 of EPCRA, which includes the reduction and recycling information required by Section 6607 of PPA.
(6) The toxic chemical and hazardous substance release and use reduction goals of section 2(e) of Executive Order 13423 and of Executive Order 13514.

C.7 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2012)
(a) Definitions. As used in this clause—
(1) Contract financing payment has the meaning given in FAR 32.001.
(2) Designated agency office has the meaning given in 5 CFR 1315.2(m).
(3) Electronic form means an automated system transmitting information electronically according to the Accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests.
(4) Invoice payment has the meaning given in FAR 32.001.
(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.
(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.
(c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following:

(2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI). The X12 EDI Web site (http://www.x12.org) includes additional information on EDI 810 and 811 formats.
(d) Invoice requirements. Invoices shall comply with FAR 32.905.
(e) Exceptions. If, based on one of the circumstances below, the contracting officer directs that payment requests be made by mail, the contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office. Submission of payment requests by mail may be required for:
(1) Awards made to foreign vendors for work performed outside the United States;
(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;
(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;
(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or
(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

C.8 VAAR 852.237-70 CONTRACTOR RESPONSIBILITIES (APR 1984)
The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of Washington. Further, it is agreed that any negligence of the Government, its officers, agents, servants and employees, shall not be the responsibility of the contractor hereunder with the regard to any claims, loss, damage, injury, and liability resulting there from.
