Questions and Answers – Laser Services Question 1: I am hoping to find out if there is an incumbent contractor currently performing these services or if this is a new requirement. If there is an incumbent, could you please provide the contract number?

Answer 1: There is an incumbent contractor although its contract does not specify as many services. The contract number is VA260-15-P-0002.

Question 2: Specifically what technology is needed to complete the requested cases? For instance, do the Holmium cases need to be done on a 60 watt or higher laser?

Answer 2: We have listed the laser technologies that have been specifically requested by our surgeons; however, we wish to keep the list intentionally open to allow for new technologies, or technologies that our surgeons have not previously considered. In light of changing technologies and updated practices, our surgeons would prefer to review what is available for use and at what levels these technologies are available.

Question 3: ESWL is listed in the notes but not in the case quantities on page 62, how many cases if any are included for ESWL?

Answer 3: ESWL is now included for anywhere from zero to approximately 3 per year – this is for backup purposes only. Should the VA’s new equipment fail, then more cases would be requested.

Transmyocardial Laser revascularization (TMR), qty 60 is also added to the Price Worksheet and PWS.

Please use the attached revised D.2 PRICE WORKSHEET for your quote.

Performance Work Statement (PWS) paragraph 1.0 is changed to include ESWL and TMR.

1.0 Scope of Work: Surgical Services at the Puget Sound Health Care System (PSHCS) requires a contractor to provide rental of: ESWL, Cyber TM High Power (Thulium), Cyber TM Scope, Holmium Laser, Microsurgical KTP Laser, Omniguide (brand name or equal) CO2 with additional fibers, CO2 Laser, GreenLight (brand name or equal) Laser, MoXy (brand name or equal) fiber, Continuous Flow Cystoscope, ESWL, and or Transmyocardial laser revascularization (TMR), with a technician to run the equipment and to provide technical support during the procedure. Laser services not listed above will be considered within the scope of this contract and may be added via contract modification by creating additional sub Contract Line Item Numbers (CLIN) and prices; this will be negotiated between the VA Contracting Officer and the contractor before these potential additional laser services can be ordered.

Question 4: Per page 73 Part III, “Note” it appears we will only bill for services rendered, please verify this will be applied for services rendered below and above the estimate.

Answer 4: Yes. This solicitation will result in a VA Indefinite Delivery Indefinite Quantity contract (Solicitation, page 1, Block 20.) The quantities of services are estimates only and are not purchased by the contract.

Question 5: By nature, surgical cases can only be estimated and are not exact. How do we guarantee we only bill for cases done if we are below what is estimated or bill for more cases if we are above what is estimated?

Answer 5: Solicitation Section B.5 contains the contract Minimum and Maximum.

Question 6: Different types of procedures can require a wide range of disposable items per case. How would we include these items in this RFP as each case, even if done with the same technology, may need different disposables?

Answer 6: The VA understands the need for disposables during laser procedures. Disposables will be covered under a separate agreement with the VA Logistics Service line after award. After award, the VA Contracting Officer’s Representative (COR) will require a full listing with cost information for all potential disposable items. These items will then go through a one-time vetting process prior to coming in to the hospital. The contractor’s Laser rep will be required to provide potential disposables at time of service. The VA will not stock or consign these disposables. Disposables will not be included as a billable item on this contract but will be invoiced separately.
