Shipping Boxes and Invoice Paper contract
2. Background
The Department of Veteran Affairs Boise VA Medical Center, Boise ID (VAMC) has an ongoing program to furnish eyeglasses to Veterans. It is anticipated that the VISN 20 Optical fabrication Laboratory (VISN 20 OFL) will manufacture and produce an estimated (EST) 50,000 to 150,000 pairs of ophthalmic eyewear per annum with an EST average of 7,000 pairs per month. The quantities identified herein are EST only. The actual quantities will be based upon the number of patients who are determined eligible by the VA to receive ophthalmic eyewear.
3. Scope
3.1 The contractor will provide shipping boxes to the VISN 20 OFL for use to mail completed eyeglasses in cases to Veteran Patients throughout the VISN 20 catchment area.
3.2 The contractor will provide adhesive peel away invoice paper to the VISN 20 OFL for use as work tickets to manufacture Veteran eyeglasses.
4. Specific Tasks
TASK 1:
4.1.1 The contractor will provide shipping boxes, in the dimensions listed, assembled, with an auto-bottom feature to the VISN 20 OFL. Dimensions are 7 ¼ inches by 3 ¼ inches by 2 ¼ inches. EST quantity: 84,000 each of boxes per year. See Pictures attached.
TASK 2:
4.1.2 The contractor will provide Invoice paper, in the dimensions and specifications listed, shipped to the VISN 20 OFL. Dimensions and specifications are 8 ½ inches by 11 inches plain paper with adhesive strip on bottom, #20 Integrated Label 1000 sheets per box. EST quantity: 180 boxes of 1000 per year. Pictures attached.
5. Invoicing and Billing
5.1 An itemized invoice shall accompany each filled order from the contractor. This invoice must include invoice number, PO number, Order Date, Ship Date, Description of items, and show quantities of each item ordered, shipped, and backordered, as well as item cost and item extended cost. The itemized billing invoices will be submitted to the VA Financial Services Center (VAFSC) for payment through Tungsten Network for payment no later than 10 working days after each month of service. Billing invoices can be submitted sooner if desired by the Contractor. All invoices will be submitted in accordance with these instructions.
5.2 In 2007 the Department of Veterans Affairs (VA) entered a relationship with Tungsten Network. With Tungsten Network, all Department of Veterans Affairs suppliers will now be able to securely submit invoices directly from their existing invoicing formats. In addition, by submitting invoices electronically through Tungsten Network, Department of Veterans Affairs suppliers will experience prompt payment with reduced exceptions.
5.3 Your participation in this existing process is required to continue providing products or services to the Department of Veterans Affairs in compliance with the Improper Payments Elimination and Recovery Act of 2010 (IPERA) and mandate AN97. The rule mandating electronic invoice submission was published on November 27th, 2012, and has an effective date of December 27th, 2012. Mandate AN97 can be located on the Federal Registry at https://www.federalregister.gov/documents/2012/11/27/2012-28612/va-acquisition-regulation-electronic-submission-of-payment-requests .
Vendors can comply with the mandate by using either of the two methods below:
1. The Financial Services Center (FSC) uses a third-party contractor, Tungsten Network, for electronic invoice submission. For information on Tungsten Network electronic invoicing set-up, vendors should call 866-340-4980 or email http://www.tungsten-network.com/veteransaffairs/ .
2. A system that conforms to the X12 electronic data interchange (EDI) format established by the Accredited Standards Center. For FSC e-Invoicing information, please call 877-353-9791 or email vafsccshd@va.gov .
5.4 Any billing discrepancies shall be addressed with the vendor for resolution within 15 days from the receipt of the comprehensive bill.
5.5 The VA may dispute a charge if an invoice has not been received, or it does not match the contracted price or quantity received. If VA disputes a charge, that charge will be deducted from the invoice amount to be paid and the contractor notified accordingly. The invoice must consist of fabrication and dispensing. Therefore, the contractor may provide proof of shipment with a proper invoice listed above. Payment can be made only after certification from the COR or designee that the prices billed are correct.
6. Shipping
All shipping should be sent by local carrier at the contractors’ expense (FOB: Destination). In the event the Government desires expedited delivery, the ordering activity shall telephonically contact the contractor and inquire into the feasibility of obtaining this condition. The contractor shall respond within 24 hours to such request and expedited delivery shall be mutually agreed upon. Upon direction of the VA, delivery of all shipping boxes and Invoice paper will be delivered directly to the VISN 20 OFL at 500 W. Fort Street, BLDG 28, Boise, ID 83702, ATTN: VISN 20 OFL.
7. Performance Monitoring (if applicable)
The contractor will deliver shipping boxes fully assembled and boxed and Invoice paper to the VISN 20 OFL. The contractor’s performance will be monitored from when an order for supplies is made until the delivery of those supplies to the VISN 20 OFL address listed in para. 6 above.
8. Security Requirements
The Contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. No background investigation is required.
9. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI)
Dyes already in use may be transferable. Verification upon award. Please include Dye fees in your Quote in case dyes are not transferable.
10. Other Pertinent Information or Special Considerations
a. Identification of Potential Conflicts of Interest (COI)
VISN 20 OFL has already paid for the dye charges for the designed invoice paper label location layout with the VISN 20 OFL needs. Rotary Dye is currently located at current manufacturer and may be transferable. The current vendor owns the Dye for the boxes. Although not proprietary these charges have already been paid for by the VISN 20 OFL.
b. Inspection and Acceptance Criteria
All shipping boxes and Invoice paper will be new. The cases that hold the shipping boxes can be reused until no longer serviceable at which point they will be exchanged by the contractor with new cases.
