STATEMENT OF WORK FOR

Lean Six Sigma Training

Office of Integrated Veteran Care (IVC) Portfolio Management Directorate (PMD)

Introduction:

Contractor shall provide all necessary Lean Six Sigma Black Belt certified instructors, materials, and expertise to facilitate instructor-led computer-based training and resulting Lean Six Sigma certifications for a maximum of 20 classes for designated participants in each of the following categories.

· White Belt –10 participants each

· Yellow Belt –10 participants each

· Green Belt –10 participants each

· Black Belt –10 participants

· Master Black Belt – 5 participants

· Lean Fundamentals – 20 participants each

· Root Cause Analysis – 20 participants Languages The contractor will provide materials in English and the facilitator will be able to communicate fluently in English to match the needs of the training participants.

Facilitators The contractor shall provide one specialist facilitator and ensure he/she meets the minimum qualifications listed below. The Contracting Officer Representative (COR) will approve all individual facilitators before each teaches her first session under the contract, and request replacements if required. If a facilitator meets all minimum criteria for multiple topics, he or she may deliver those multiple topics.

The contract may require additional facilitators if more than once class is scheduled for the same time period.

· Facilitator shall be Lean Six Sigma Black Belt certified with at least 10 years teaching or facilitating Lean Six Sigma trainings. Preferred experience working with federal agencies.

Facilitator Expectations

· Each facilitator shall present information in a clear and concise manner, explaining the content to ensure all learners understand the key topics listed in the training material.

Facilitator Scheduling

· The contractor shall ensure that all facilitators are ready to present virtually at the scheduled date and time.

Logistics and Review

· Session evaluations: The COR is responsible for all session evaluations. The Project Authority will collect participants session evaluations and will review the evaluations it collects from participants and, when necessary, provide its feedback to the Contractor.

· Auditing of sessions: The COR reserves the right to audit sessions held to assess the session content and review facilitator performance.

Facilitated On-Line Distance Learning Delivery (Webinar) Sessions –

· Session timeframes: Between the hours of 8:30-4:30pm ET, for a total of not more than 8 hours per day, including 15-minute breaks (morning and afternoon) and 45-minute lunch.

· The Contractor will deliver the training through a facilitated on-line real-time method, using a webinar computer application. This will let learners participate from their workstations, see and hear presentation information in real-time, and ask questions in real-time. All technology used in course content and delivery shall comply with VA connectivity and security standards.

COR to contact Contractor, in writing, to request a session
At least 20 business days before the requested session start date
COR to submit Participant List, in writing, to Contractor (the Project Authority will aim for a minimum of 10 and maximum of 20 participants per session)
At least 5 business days before scheduled session start date
COR to notify Contractor, in writing, to cancel a scheduled course session
At least 15 business days before scheduled session start date, with no charge. The Government will pay for any course cancelled within 14 days.
Contractor shall e-mail the COR a copy of the Attendance List (must include training title, session dates, Contractor name, participant names and participant signatures)
Not more than 30 business days after the end of the session

COR can send request to Contractor, in writing, for a replacement facilitator
At any time during the contract, with at least 10 business days’ notice for the requested facilitator change to take effect
Contractor can ask to introduce a new facilitator, by sending a written request for approval to the COR
At any time during the contract, with at least 10 business days’ notice for the Project Authority to respond to the request

IT Compliance Prior to termination or completion of this contract, contractor/subcontractor must not destroy information received from VA, or gathered/created by the contractor in the course of performing this contract without prior written approval by the VA. Any data destruction done on behalf of VA by a contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. Self-certification by the contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the contract.
