STATEMENT OF WORK FOR
TELE-SITTER ANNUAL LICENSE
Background The purpose of the TeleSitter System is to use state of the art technology to ensure the Oklahoma City VA Health Care System provides the highest level of safe and quality patient care. The TeleSitter System will provide the ability to have uninterrupted, remote monitoring and interactions with individual patients therefore reducing fall risks with integrated alarming measures and two-way instant communication.
Objectives/Scope The TeleSitter System Software purchase will provide new annual licensing for the program we already have installed. The vendor will provide the license use in the VA medical center. The contractor shall furnish the license and all updates to the system already in place.
Selection Criteria 12 Software Licenses
Product Specifications for: Tele-Sitter System
Quantity: Avasure Software standard 1 year licenses for 12 devices. Base + 4 year options.
Deliverables or Delivery Schedule The contractor shall deliver the licenses via upload to current program.
The equipment shall be delivered to the following address:
OKC VA Health Care System (Bio-Medical Service) 921 NE 13th Street
OKC, OK 73104
Documentation necessary includes a statement acknowledgement/invoice and receipt of delivery.
Place of Performance The place of performance will be through the entire Oklahoma City VA Health Care System in Oklahoma City.
Period of Performance The contractor will have 30 working days to complete installation of the system after the delivery date is established with the COTR.
