STATEMENT OF WORK

A. GENERAL INFORMATION

1. Title of Project: Sheridan Veterans Affairs (VA) Health Care Center (VAHCS) Physical Access Control System (PACS) Replacement

2. Scope of Work: The Sheridan VAHCS will purchase (1) new Lenel-S2 Enterprise door access system controller with 256 door licenses. The system will also include (48) nodes and (66) access blades. The Sheridan VAHCS will re-use all existing wiring, card readers, door position switches, strikes, and request to exit devices. The system will utilize contacts from the existing duress system to initialize lockdown features and will be programmed to secure areas only allowing first responders to access and clear the buildings. The sizing of this system is based on the existing 151 doors. The equipment will be shipped directly to Sheridan VAHCS. Sheridan VAHCS will request four subscriptions to Lenel-S2 online training that will allow staff to perform the same training as installation technicians receive. Upon completion of all PACS hardware and software, the training modules will be open for one year to be accessed as many times during the year as needed.

The vendor will remove the existing door access system cabinets, install the new Lenel-S2 cabinets, nodes, and access control blades. The existing wiring will be disconnected from the old system and re-connected to the new Lenel-S2 system and tested. Any readers, door position switches, strikes, or request to exit devices that fail will be replaced under this contract. No new doors will be added to the system under this contract. The vendor will program the Lenel-S2 system for Sheridan VAHCS to include access levels, forced door alarms, event alarms, threat levels, and reporting. During training Sheridan VAHCS will be instructed on utilization of the system, install card holders, create schedules, and run reports. The vendor will take a dry set of contacts provided by the company that provides the duress system to the door access system for lockdown functionality and program it as requested by Sheridan VAHCS to secure the facility as needed and allow first responders to clear the buildings.

The vendor will supply a one-year maintenance and support option for the system equipment, card readers, strikes, door position switches, request to exit devices, power supplies, programming, and the system licenses with no cost to Sheridan VAHCS. During this time frame the vendor will update the system licenses, provide programming support both on-site and by phone as needed, and replace defective hardware. Sheridan VAHCS requires a 2.5 hour onsite response timeframe for live service and maintenance requirements as needed during vendor business hours.

3. Background: Sheridan VAHCS is requesting a new control access system to maximize employee accountability, patient safety and protection. The current PACS system does not meet the standard of Sheridan VAHCS needs thereby creating the necessity for a system upgrade replacement. The procurement is for PIV servers and equipment, onsite installation and coverage testing, training, testing, one-year annual preventative hardware and software service maintenance with a 2.5-hour onsite response time during vendor business hours. The contract and equipment will include all installation services, equipment, maintenance, training for new and existing staff, and technical consultation. All PACS PIV servers and subsequent equipment will be tested upon completion of project.

4. Performance Period: This contract is a one-time equipment and installation procurement, full annual preventative service and maintenance plan, and quarterly onsite inspections.
