1) B.5 Price/Cost Schedule

2) E.2 Addendum to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS (JUN 2020)

3) PRICE SCHEDULE BPA RFQ 36C25921Q0389 0001 (separately attached)

All other items remain unchanged.

Monica Gary Contracting Officer See attached document: PRICE SCHEDULE BPA RFQ 36C25921Q0389 0001.

B.5 PRICE/COST SCHEDULE

The Contractor shall provide pricing for three or more orderable items in the Price Schedule spreadsheet (Price List) attached in Section D.

The VA prefers unit prices are inclusive of shipping charges and all other associated costs for a smooth and seamless transaction process by making it less onerous on the VA during the comparison of quotes prior to issuance of a funded task order. If there is no separate price schedule of itemized shipping charges the VA will accept that the contractor’s shipping charges are included in the unit prices in the spreadsheet Price List (Price Schedule).

The prices included on the BPA list (or applicable "discounted" rates submitted in a proposal response to an RFQ resulting in award) that are in effect on the effective date of an order shall govern that order's basic performance period. With regard to any option years, which are later exercised, the proposed option year rates as incorporated into the order award are in effect until such time, if applicable, that the contractor has been authorized a rate increase culminating from a negotiation with the Contracting Officer.

Price Reductions: The BPA holder can voluntarily reduce offered prices at any time by giving 24-hour advance notice (by electronic-mail) to the Department of Veterans Affairs/Contracting Officer. This BPA also allows for additional discounts if a "large order" is placed at one time. An advanced notice is not required for discounts offered for only an individual order. Whether an order is large enough to warrant such a discount is subject to the discretion of the BPA holder.

Price Increases: The BPA holder may also increase BPA prices at any time. Any BPA price increase shall not take effect until the Department of Veterans Affairs Contracting Officer receives written notification (U.S. mail, facsimile, or electronic-mail). Any order already issued shall not be affected by any change to BPA pricing. The prices offered under this BPA will undergo annual review by the Department of Veterans Affairs Contracting Officer.

E.2 Addendum to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS (JUN 2020) The Department of Veterans Affairs, Network Contracting Office 19 (NCO 19) is seeking to award a five-year Blanket Purchase Order (BPA) to one or more responsible offerors to provide Water Sample Lab Testing Services for the Sheridan VA Health Care System, Sheridan, WY.

1. Vendors must use PRICE SCHEDULE in Section D to provide their quote. Vendors are encouraged to quote as many items as possible but must quote at least three items or more per BPA Pricing Period.

If service cannot be satisfied exactly as required in the PWS, to include shipment and all associated costs, then this will be considered a NO BID item on the offeror’s quote. Vendors may provide a separate price schedule, in addition to the spreadsheet Price List, so that they may clearly identify their itemized shipping charges on a per line item basis.

If a caveat note is placed next to the quoted price this will be considered a NO BID item on the offeror’s quote.

Quotes for part of the Price Schedule may be submitted by vendors. Vendors may indicate “NO BID” against the specific line item in the Price Schedule that they cannot satisfy in accordance with the PWS and Price Schedule.
