· Real-time Outpatient Cardiac Telemetry Device - 11 to 30 days.

· We are not familiar with a service or ICD code that provides “11-30 days” of monitoring in a “real-time” format. Could you please provide some clarification or additional information for this statement?

Just means we need a “30-day monitor” that we could pull from patient earlier than 30 days if needed, but greater than 10 days.

· Holter Monitoring 24-48 hours (with Real-time Outpatient Cardiac Telemetry)

· Could you please clarify if this volume is for only two annually and further define “Outpatient Telemetry”?

Cardiac telemetry is transmission of cardiac signals. Real time is the importance here. Yes. The volume is low for these.

· "Cardiac Event Monitoring with Telemetry: 1-30 Days Cardiac Event Monitoring with Telemetry Transmitting and Analyzing (real time event monitoring only)

· Is statement “near real-time” acceptable rather than the term “real time monitoring”? This terminology is an important distinction and is based on FDA guidance. Or does this statement simply refer to “up to 30” days of telemetry monitoring which is the standard description for this service?

If the individual is in an area with good service, it will be real time. Near real time would be if the patient was in a room with bad cell reception service. Which would be acceptable so long as the monitor had access to real time monitoring when getting in a good service area. We currently use a 4g monitor (it uses the cell service)
