1) The solicitation lists services required for 2.36 FTE, which is defined as 80 hours every two weeks, however the pricing only lists 3875 hours per year. At 40 hours per week the total hours should be 4,908.80. It should be 1.86. It is changed in Amendment 1
2) The solicitation also lists “on-call” coverage, what is the expected call rotation and should we add CLIN’s to the pricing for weekday and weekend call rates? No CLINs are in there for Call. The call would be divided fairly among the Contract and FTE Providers. Add CLIN 0002 to base and all options on Amendment 5
3) Will the VA consider proposals with less than 8 FTEE key personnel? No
4) Page 14, section 2. Qualifications in subsection 2.1.1. indicates a State of CO medical license is required. Please confirm as it is unusual for Federal Contract opportunities to limit the medical license to the state of performance. Yes this is correct
5) Page 14, section 2 Qualifications in subsection 2.1.2. indicates BLS required. However, on page 15 in section 2. Qualifications in subsection 2.1.6. indicates BLS and ACLS required and is also listed in Instructions to Offerors to include ACLS. Please clarify if both BLS and ACLS are required? BLS is required by all
6) On page 18, section 2.7, No Employee Status indicates Worker's Comp is required, however we utilize Independent Contractors that are not subject to Worker's Comp. Or, will page 52, Section C.12 Supplemental Insurance Requirements apply to the Independent Contractor's Workman's Comp issue? Yes that is correct If not, how is this Workman's Comp requirement handled?
7) Page 23, section 4.4.2.1.2. indicates a list of types of Operative Services to be performed (not limited to...) however there is no list of the most common types of surgeries performed. Can you include a list of most common types of surgeries? Our most prevalent is Total Adult Reconstruction.
8) Will there be any Resident Supervision duties? No If so, how many Residents accompany the ORS during rounds?
9) Are there clinical support personnel available? If so, how many RN's, Nurse Practitioners, Physicians Assistants, or other clinical personnel, etc...are available per shift? We do have PAs that assist the surgeons.
10) How many VAMC Staff Orthopedic Surgeons are on schedule or is this opportunity structured to provide the only Orthopedic Surgeons that will be at facility? It depends . Currently we have 2.5 FTEs on board and are working toward a total of 5
11) Page 52-53, Section C.16 through C.18 regarding VA Small Business Contracting Plan indicates this applies to companies required to have a subcontracting plan in this instance, however it also states that the requirements are to be met according to the VA Goals. My understanding is the VAMC is now following the SBA Goals which are different for this requirement than the VA goals. Please clarify. It will be the VA goals which are sent to SBA.
12) Section D Attachments 1 thru 5 do not appear to be included? Added to Amendment 1
13) Is this a new contract? No the incumbent is University of Colorado and Current Contract Number is 36C25918C0265.
14) Will Government Furnished Equipment (GFE) be provided? No
15) Also does it ok if the Project Manager work remotely? Depends on your Staffing
16) Will set aside remain the same because we are not a SDVOSB/VOSB. This is unrestricted requirement
17) How many key personnel resumes (providers) should we submit with the proposal? 8
18) Will the Government accept 1 resume for proposal submission? We will need resumes from all 8 individuals
19) Is there a listing of procedures/duties that the Ortho Surgery will be expected to do? Procedures of a Joint/Tumor/Spine/Hand Surgeon. There is also call and Charting,notes, etc…Things they would do in their current setting. TMS is also in the contract!
20) What is FAR 52.212-3? http://farsite.hill.af.mil/ This is mentioned in Section E.1 (8). It states that we must provide a completed copy of the representations and certifications at FAR 52.212-3, so was wondering if you could provide clarity on what that means. Is that simply indicating that it’s the third iteration of the RFP?
4.1201 Policy. (a) Offerors and quoters are required to complete electronic annual representations and certifications in SAM accessed via https://www.sam.gov as a part of required registration (see FAR 4.1102).
21) Do we need to provide a Unique entity identifier or be registered in the System for Award Management? Your DUNS will be the Unique Identifier
22) Do you need a TIN? Yes
23) Is there anything that must be signed or completed for VA Directive 1663? No
24) What is a VETS 4212 Report and where can I find it?

25) Is the NDA and Statement of Financial Interest the same thing as the “Signed Conflict of Interest Memo from Section D?” If not, do you know where I can find the Signed conflict of interest Memo? Yes it is the same thing
