Southern Arizona VA Health Care System (SAVAHCS) Tucson, Arizona Facility Wide Enhanced Locating Upgrade

DESCRIPTION:

The Southern Arizona VA Health Care System (SAVAHCS) has an Inpatient Wide requirement to upgrade the existing Hill-Rom Nurse Call Standard Locating System with the new Hill-Rom Navicare Enhanced Locating platform located in the following areas:

Building 38 ICU

Building 38 Ward 3 East

Building 38 and Building 57 Radiology

Building 50 Emergency Department

The scope of work for this installation will include de-installation of existing modalities; as well as the material, parts, software, labor, and certification of a completely operational and functional system that will interface to the enterprise Nurse Call network. This is a BRAND NAME ONLY requirement.

MATERIAL AND INSTALLATION:

SAVAHCS will purchase the necessary upgrades for the Hill-Rom Navicare Enhanced Locating System that will integrate into the existing facility-wide Hill-Rom Nurse Call Network. SAVAHCS utilizes the Hill-Rom Nurse Call Network for the life safety Code Blue features throughout the facility.

All clinical areas throughout the facility that utilize nurse call have been standardized to the Hill-Rom solution and the new Enhanced Locating platform will interface all area Power over Ethernet (POE) switches into the enterprise network.

In general, listed in the attached BOM are standard Star Timing Devices, Room Monitoring devices, Power over Ethernet (POE) switches, and installation services for each area that will be upgraded to the Enhanced Locating platform listed in the DESCRIPTION section above. Installation services will include cable pulls, terminations, and interface programming to the existing Hill-Rom Navicare Nurse Call network by the vendor.

The equipment which is the network infrastructure for integrating all areas that will be upgraded to the Enhanced Locating System. The topology of the Navicare network involves local servers that communicate to the Enterprise server for central processing and distribution of data, alerts, and information. This section provides the programming, cabling, hardware, and software for complete and comprehensive integration.

PERFORMANCE:

· Hours: Normal duty hours are 8:00 a.m. through 4:30 p.m., Monday through Friday, not including federal holidays. Whenever contact with Biomedical Engineering is required outside of normal duty hours, the Utility System Repair Operator (USRO) may be contacted as an alternate. The hospital operator at extension "0" can contact the USRO operator via radio.

· All contractor personnel (without exception) must report to the Biomedical Engineering Department, Bldg. 38-room A22, to sign in and receive a contractor identification badge before any work is performed and return to the same location to sign out and turn-in said badge along with the documented service report, as applicable, for the service performed.

· Noncontract Charges: The contractor shall not perform any service that will result in additional charges without prior approval from the Contracting

· Payment: Payment will be made in arrears, and upon receipt of a properly prepared invoice which references the period for which payment is due, the contract number and/or assigned purchase order number. The invoice must include the correct payment address.

· Waste Removal: The contractor is responsible to remove and properly dispose of all waste related to the services provide on-site at SAVAHCS.

· Failure to carry out the above procedures will be considered as a failure to document the service visit and will constitute a failure to perform.

· All shipping for parts will be included in the cost of the contract.

· VA property may not be removed from the SAVAHCS prior to execution, and approval of all documents as required by Acquisition and Materiel Management Service. Computer hard drives and other storage media items may not leave VA property.

The integrator shall comply with all the following:

ANSI/EIA/TIA 606-A (Administration Standard for the Telecommunications Infrastructure of Commercial Buildings)

ANSI/EIA/TIA-607 (Grounding and Bonding Requirements for Telecommunications in commercial Buildings)

National Fire Protection Agency (NFPA) LIFE SAFTEY-101

National Electric Code (NEC)

The integrator shall comply with all the following that apply:

All horizontal penetrations through smoke and/or fire rated walls shall use STI EZ path (EZDP44S). (The SAVAHCS has standardized on this type of fire penetrating sleeve)

All Voice and Data Telecommunication Outlets shall be terminated using the 568-A color code

All Optical Fiber cable (Single-mode, Multi-mode) shall be terminated using LC connectors

All Optical Fiber cable shall be installed and terminated in existing (Line Interface Units) LIUs where applicable

All new LIUs provide by the integrator shall be Leviton Opt-X Ultra 4RU (The SAVAHCS has standardized on this type of LIU)

All LIU bulkheads provided by the integrator shall be 24 port high density LC style bulkheads

All Multi-Mode Optical fiber provided shall be 50um Laser Enhanced OM3 and/or OM4

All Voice station cable provided shall be WHITE (in color) CAT 5e or better All Data station cable provided shall be BLUE (in color) CAT 5e or betterAll Data WAP cable shall be GREEN (in color) CAT 5e or better

All Data CCTV, P-TUBE, and LENEL (3300, 2220) cable shall be ORANGE (in color) CAT 6 or better

All station location faceplates shall be IVORY (in color). The SAVAHCS has standardized using Leviton.

All Voice jacks provided at the station location shall be CAT 5e minimum and IVORY (in color)

All Data jacks provided at the station location and the corresponding patch panel shall be BLUE CAT 5e minimum (in color)

All Data jacks provided at WAP locations and corresponding patch panels shall be CAT 5e minimum GREEN (in color)

All Data jacks provided at CCTV, P-TUBE, and LENEL (3300, 2220) locations and corresponding patch panels shall be CAT 6 minimum ORANGE (in color)

All Telecommunication Station locations shall use a minimum of a 4 Square box with a Single Gang Reducer plate

All Station faceplates shall be 6 port, single gang, IVORY (in color)

All non-used station faceplate ports shall have IVORY (in color) blanks inserted.

All Station faceplates shall have the Voice jack installed in the top left port of the faceplate, Data A installed in the left middle port of the faceplate, and Data B installed in the bottom left port of the faceplate. If all 6 ports are used, then the second set of station cables shall have the Voice jack installed in the top right port of the faceplate, Data A installed in the right middle port of the faceplate, and the Data B installed in the bottom right port of the faceplate

All cable terminations in the telecommunications closet are to be terminated and labeled in numerical order.

All data terminations labels used on the patch panels are to follow the labeling scheme of the voice location.

Labeling Schema for all Voice, Data, Waps, and ancillary cabling shall be provided to the integrator by the SAVAHCS Electronics Section

All relay racks are to be black (in color) 7’ 19” four post (SAVAHCS standard) All relay racks installed are to have 4 matching black bi-directional vertical wire-mangers

All relay racks shall have a bi-directional 2 Rack Units (RU) horizontal cable managers installed above and below all installed patch panels

All patch panels shall be 2RU unpopulated 48Port.

All patch panels shall have the station data ports A and B installed side by side

An unpopulated patch panel shall be provided and designated for ancillary equipment only (WAP locations, CCTV locations, Patient Med Flow System, etc…)

All relay racks, ladder trays, and other installed modalities required by ANSI/EIA/TIA 607, shall be bonded to the Telecommunications Main Ground Bus bar (TMGB)

All CATV equipment shall be connected and balanced into the existing SAVAHCS CATV 870MHz system.

All CATV station locations shall be balanced between 0dBmV to 12dBmV (optimal 6dBmV)

All CATV equipment used shall be Blonder Tongue. (The SAVAHCS has standardized using Blonder Tongue head end equipment, amplifiers, taps, and splitters.)

All CATV amplifiers used shall be both bi-directional and power doubling

All CATV amplifier outputs shall be sloped to 36dBmV/ 44dBmV

All CATV f-connectors shall be compression style.

All CATV Fiber connectors shall be LC single-mode angle polished.

CATV design shall utilize a star-topology for installation instead of using the home run approach.

All CATV amplifiers, taps, and splitter shall be grounded

All PA speakers shall be Bogen (The SAVAHCS has standardized using Bogen SM4T speakers) surface mount speakers with volume attenuating control knobs.

All PA amplifiers shall be Bogen 70V systems.

All PA amplifies shall have a Matching Transformer installed for use with 600-ohm balanced audio (The SAVAHCS has standardized using a Bogen WMT1A Matching Transformer) All PA speaker audio volume levels shall be balanced using an audio signal meter for optimum audio coverage based on the areas they are installed.

Integrator shall be a Cisco Certified Partner when adding to or modifying the SAVAHCS’s CCTV systems

Integrator shall install, program, and test all CCTV cameras into the existing Cisco Video Surveillance Operations Manager (VSOM)

All CCTV locations shall have Orange (in color) CAT 6 Unshielded Twisted Pair (UTP) cable

All CCTV locations shall have Orange (in color) CAT 6 jacks installed at both the camera location and the communication closet location

All CCTV cameras utilized shall be TCP/IP and support Power Over Ethernet (POE)

All Interior CCTV cameras shall be IP Domes with Vari-focal lenses

All CCTV cameras shall be Day/Night with motion detection and event notification capabilities

All CCTV cameras shall be capable of doing 1080p at 30frames per second using H.264 compressions (SAVAHCS Standard)

All CCTV cameras shall support dual streaming

Integrator shall be a Lenel Certified Partner when adding to or modifying the SAVAHCS’s Physical Access Control System (PACS) systems

Integrator shall be a Litehouse Certified Partner when adding to or modifying the SAVAHCS’s Physical Security Information Management (PSIM) system

All PACS locations shall have minimum 16AWG/4 Conductor cable for lock power

All PACS locations shall have minimum 22AWG/6 Conductor cable for card readers.

All PACS locations shall have minimum 22AWG/2 Conductor cable for door monitor switch. (Door Contact)

All PACS locations shall have minimum 22AWG/4 Conductor cable for request-to-exit device. (RTE)

The SAVAHCS has standardized PACS cabling using Belden Banana Cable (B658GFS)

All door locations retrofitted with access control modalities shall maintain a distance of no less than 78” (6’8”) between finished floor and the top of the door jam.

PACS electric strikes shall be HES 1006 series with KM faceplates or Comparable. (This is the SAVAHCS standard lock).

PACS magnetic locks shall have a holding force no less than 1500lbs.

If magnetic locks are installed the integrator is required to install, program, and test Request to Exit motion (RTE), connection to Fire alarm, and a blue glass break emergency egress device that directly interrupts power of the maglock (SAVAHCS Standard is SDC 491)

PACS locations that use magnetic locks shall have an emergency bypass switch installed within 5 feet of the egress side of the door at ADA required height (40 to 48” from finished floor) for emergencies

PACS audible devices shall have a decibel rating no less than 85db.

PACS motion detectors used shall use both Passive Infra-Red (PIR) and Microwave technologies for detection

PACS readers shall be RPK 40 Keypad Readers. (SAVAHCS has standardized using RPK 40 HID Keypad Readers 921PHRNEK0002D)

PACS modalities shall be Lenel (LNL-1320, LNL-1300, LNL-1100, LNL-1200, LNL-3300, LNL-CK, ETC…)

PACS door monitor switches (Door Contacts) shall be flush mount and brown in color.

PACS power supplies shall be equivalent to or exceed the specs of the Von Duprin 914 power supply with an installed 4 option card, and fire alarm card

PACS power supplies shall be installed within 5 feet of the access door being locked, and have a pigtail for plugging into a 110 receptacle

Integrator shall provide programming, and testing of all PACS, and CCTV devices into the existing Physical Security Information Management (PSIM) (The SAVAHCS currently uses Litehouse System for managing sensors)

Patient Med Flow System shall utilize TekTone LI382 dome lights for standardization

All TekTone dome lights shall have 2 RED (in color) LEDs Digital Display Products (DDP) (MB400-R12-4AL1) for standardization

All Patient Med Flow System cables shall be Orange (in color) CAT 5e All Patient Med Flow cables shall be terminated in the MDF using a 110-style block with standoffs and wire management

All Patient Med Flow Station location cables shall terminate the Blue pair of the CAT 5e cable to the 1st LED using splice connectors and terminate the Orange pair of the CAT 5e cable to the 2nd LED using splice connectors

All Patient Med Flow Monitoring stations shall be All-in-One PCs with touch-screen technology (CYBERNET iOne-GX-45 17”) for standardization

C.2 SUPPLEMENTAL INSURANCE REQUIREMENTS

In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:

(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.

(b) General Liability: $500,000.00 per occurrences.

(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.

(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.

C.4 VAAR 852.212-71 GRAY MARKET ITEMS (APR 2020)

(a) No gray market or remanufactured items will be acceptable. Gray market items are Original Equipment Manufacturers’ (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical equipment only for VA medical facilities.

(b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions.

C.5 VAAR 852.212-72 GRAY MARKET AND COUNTERFEIT ITEMS (MAR 2020)

(a) No used, refurbished, or remanufactured supplies or equipment/parts shall be provided. This procurement is for new Original Equipment Manufacturer (OEM) items only. No gray market items shall be provided. Gray market items are OEM goods intentionally or unintentionally sold outside an authorized sales territory or sold by non-authorized dealers in an authorized sales territory.

(b) No counterfeit supplies or equipment/parts shall be provided. Counterfeit items include unlawful or unauthorized reproductions, substitutions, or alterations that have been mismarked, misidentified, or otherwise misrepresented to be an authentic, unmodified item from the original manufacturer, or a source with the express written authority of the original manufacturer or current design activity, including an authorized aftermarket manufacturer. Unlawful or unauthorized substitutions include used items represented as new, or the false identification of grade, serial number, lot number, date code, or performance characteristics.

(c) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions.

(End of Clause) C.6 VAAR 852.219-74 LIMITATIONS ON SUBCONTRACTING—MONITORING AND COMPLIANCE (JUL 2018)

(a) This solicitation includes FAR 52.219-4 Notice of Price Evaluation Preference for HubZone Small Business Concerns.

(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement.

(c) All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the Contractor's business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs.

(d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.

C.8 52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

Technical and past performance, when combined, are price only.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e)(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—
