Solicitation 36C25820Q0188 Q&A 6/30/2020
1. Please clarify what is it that I am to do with top sheet (Answer blocks 12,17,23,24,30)? And sign where?
On page 61, Instructions to Offerors, it provides details on using the SF 1449 or a response on your company letterhead. On page 65, 1. Price Quote Content, it discusses the blocks to be completed by offerors on the SF 1449 with your signature.
2. Do not see where the quote number to be added?
If your record system requires/uses a quote number for reference, you can include that for your record keeping purposes. We require a reference of the solicitation number, noted on the SF 1449.
3. Contract P.10: d. No need to discharge if returns < 14 days? How does this effect HMIS Data entry?
After a Veteran has been absent for 3 consecutive days we can no longer pay the contractor. The contractor has the option to hold the bed without pay for up to 14 days. For HMIS, this would mean delaying the discharge note for up to 14 days. For example, if a Veteran was admitted to the hospital for 14 days and the contractor held their bed, I would stop paying them after day 3 and they would keep the HMIS admission open. If the Veteran was hospitalized for more than 14 days, the contractor would be required to submit a discharge note in HMIS and release the bed.
4. Contract P. 11-does this mean 1 Case Manager and One Case aid for total of 15 residents?
There should be no more than 15 residents per case manager or case aid. There doesn’t need to be a team (case manager and case aid) for every 15 residents. The contractor would need 2 case managers/case aids for 18 residents.
5. Contract P.17-M. Quality control Plan- is there a specific form for this?
This is a plan developed by the contracted facility. Item M. Quality Control on page 17 details the requirements of this plan (see M 1.1)
6. P 43. holidays??? We do not have this as we have less than 15 employees and go by the Arizona statutes for part time-fulltime employees.
This is language from the current Wage Determination established for Maricopa County. We include this so that potential contractors are incorporating the minimums set forth by the State of Arizona.
7. Contract P. 51- SOR- System of Records?
The contractor must have a system in place to protect the confidentiality of Veteran residents. This pertains to medical records, email transmissions, lab results, progress notes, case management records, HMIS data etc. The contractor must follow HIPPA guidelines to protect the confidentiality of Veteran residents and report all breaches to the HCHV COR.
8. Contract P.56-VA Training?
Refer to page 31 for Alternate I (JAN 2017) of 52.224-3: As prescribed in 24.302(b), if the agency specifies that only its agency-provided training is acceptable, substitute the following paragraph (c) for paragraph (c) of the basic clause: (c) The contracting agency will provide initial privacy training, and annual privacy training thereafter, to Contractor employees for the duration of this contract. If you do not have this training available you can offer the VA training to your staff.
9. Contract P.60-Is this where I sign for the actual contract?
This is part of the Rules of Behavior, the security requirement for those contractors that need access to VA Systems. If you would require this access, then yes you would complete this form as well as the VA prescribed trainings to be granted that access.
