SUPPLEMENT B
PURCHASE & INSTALLATION SERVICES – STATEMENT OF WORK
CONTENTS
I. Background II. Project Objectives III. Statement of Work/Requirements IV. Special and Technical Requirements
I. BACKGROUND
The Phoenix VA Medical center is an older facility. As we do many projects to upgrade, there has been quite a bit of asbestos found in the flooring mastic, pipe insulation, fire prevention insulation and in the texture of drywall mud joints. This request is to have an BPA in place to abate asbestos and mold when required due to impeding project completion or leaks within the structure of the facility.
II. PROJECT OBJECTIVES:
The objective of this project is to remove all asbestos and/or mold from any locations that it is found during a project or normal work order system discovery.
III. SCOPE OF WORK/REQUIREMENTS:
A. Scope of Work
1. Test suspected sample/materials to assess whether it contains enough asbestos containing materials (ACM) or mold to require remediation, by a certified inspector. If sample is taken, a report will be provided to the Phoenix VA Hospital Engineering Service Contracting Officer Representative, within 48 hours of sample being taken.
2. A certified abatement contractor will remove asbestos or mold from facility structure by cutting out drywall, removing flooring/mastic, cutting out insulation, etc.
3. A third-party contractor will perform post-clearance air sampling/procedures to ensure areas is safe once ACM or mold has been removed. Report will be provided to the Phoenix VA Hospital Engineering Service Contracting Officer Representative, within 48 hours of air clearances being complete.
4. Contractor will provide, install, remove and dispose of all infection control barriers, negative air machines, humidification, sticky mats, etc. as well as asbestos or mold debris removed from structure.
5. Contractor will follow all facility provided Infection Control Risk Assessment requirements while working on the project and on the site.
B. Hazardous Material Assessment:
All asbestos will be removed and disposed of by a certified contractor. All areas will be tested for air clearances by a third- party contractor.
C. Underground Utility Locations:
Locating underground utilities is not required for this project.
D. Commissioning:
No formal commissioning process is required.
E. Phasing and Impacts:
Project will have proper barriers and negative air machines as required. These areas will be shut down from all traffic not working on the abatement.
F. Interior Design
No interior design work is required for this project.
G. New Construction Room Numbers:
No rooms will be renumbered during this project.
IV. SPECIAL & TECHNICAL REQUIREMENTS
Asbestos Contractor/Supervisor
The Asbestos Contractor/Supervisor Initial training is for any individual who performs supervision of persons permitted to enter the restricted and regulated asbestos abatement work area. The supervisor is also responsible for performing the duties of the OSHA competent person for the asbestos project, consistent with current EPA and OSHA regulations.
Asbestos Worker/Handler
The Asbestos Worker/Handler training accredits individuals to remove, encapsulate, enclose, repair or disturb friable or non-friable asbestos, and to handle asbestos material in any manner which may result in the release of asbestos fiber on an asbestos abatement project.
| C & A Requirements | |
| The C&A requirements do not apply, Security Accreditation Package is not | |

* Waste transport. Although there are no regulatory specifications regarding the transport vehicle, it is recommended that vehicles used for transport of containerized asbestos waste have an enclosed carrying compartment or utilize a canvas covering sufficient to contain the transported waste, prevent damage to containers, and prevent fiber release. Transport of large quantities of asbestos waste is commonly conducted in a 20-cubic-yard “roll off” box, which should also be covered. Vehicles that use compactors to reduce waste volume should not be used because these will cause the waste containers to rupture. Vacuum trucks used to transport waste slurry must be inspected to ensure that water is not leaking from the truck.
